The goal of the Home HealthRegistered Nurse (RN) is to return the patient to the highest practicable level of independence and well-being within the community with self-management of disease and other identified processes.
When the highest practicable level of well-being is not independence, the goal is to prepare the patient and family/caregivers for a continuum of care within the home and/or within other community-based health care systems.
Responsibilities:
Applies nursing knowledge in assessment, diagnosis, outcomes identification, planning, implementation, and evaluation of human response to actual or potential health problems in the Home Health care setting. Initiates health teachings, health counseling, and provision of care that supports life or restores well-being. Completion of Home Health OASIS documentation in a timely manner. Communicates case findings and functions under the direction of the Nursing Supervisor, Clinical Manager, and the Director of Patient Clinical Services.

QualificationsQualifications:
- A graduate of an accredited, approved school of professional nursing
- Active license in the State of Colorado in good standing.
- Must be able to communicate in the language of the client and document client related-activity as appropriate
- Current driver’s license
- At least one (1) year of experience as a Registered Nurse. Recent Home Health experience required.
- Familiarity with OASIS preferred
- Due to the requirements mandated by the Colorado Board of Health, you must be fully vaccinated to be considered for this position with Optimal Home Care, Inc.
