Administer nursing care to patients in anefficient, safe, and courteous manner using proper nursing procedures. Supportsthe patient and family in the dying process. Performs an admission assessmentand subsequent assessments. Completes required documentation. Addresses thepatients learning ability and needs. Develops a care plan. Enters orders bycomputer and participates in the daily operation of the Acute Care Unit.Attends required in-service, annual education and mandatory departmentalmeetings. Participates in the collection of data for quality improvement.Adheres to hospital and departmental policies. Practices good communicationbetween departments, with physicians, each other, the director and clinical supervisor.The staff member must be able to demonstrate the knowledge and skills necessaryto provide care appropriate to the age of the patients served on his or herassigned unit. The individual must demonstrate knowledge of the principles ofgrowth and development over the life span and possess the ability to assessdata reflective of the patient's status. They must be able to interpret theappropriate information needed to identify each patient's requirements relativeto his or her age-specific needs, and to provide the care needed as describedin the unit's/area's/department's policies and procedures.

ESSENTIAL FUNCTIONS

Nursing Practice

1. Complete a head to toe physical assessment and asocial assessment of the patient upon admission.

2. Develop a written Care Plan upon admission andreview and update it daily.

3. Initiate cardiac monitoring, oxygen therapy,non-invasive blood pressure, and oximetry. Start/or evaluate IV site. Evaluateexisting drainage systems in place.

4. Perform delivery of nursing care via the nursingprocess: assessment (collection of data), problem identification (nursingdiagnosis), planning, nursing interventions and evaluation and updating ofplanning.

5. Re-assess patient to identify progress and trendsthat require intervention or reporting to the physician.

6. Assist patient, physician and peers in determininglong and short-range goals for patient and formulating a plan to work toward thosegoals.

7. Assist Critical Care RN with telemetry monitoringand maintain knowledge of basic arrhythmia interpretation.

8. Operate and manage equipment including thefollowing: IV pumps, PCA pumps, feeding pumps, telemetry monitors, CPAP andBi-PAP machines, defibrillators and Doppler.

9. Provide routine patient care. Perform blood drawfrom central lines. Interpret computerized monitor trends.

10. Administer medications, calculate doses andmaintain current knowledge of medications.

11. Maintain knowledge of the patient's status duringhospitalization and work with health care team to adjust and improve approachto recovery. Utilize critical thinking skills for appropriate patientinterventions.

12. Carry out and document medical and nursingtreatment, nursing care, interventions and outcomes in a timely and accuratemanner.

13. Complete documentation necessary for transitionbetween statuses, including to swing bed.

14. Assist physicians in care of the patients. Makerounds, assist with procedures, provide progress reports on patient status, andassure that orders are carried out.

15. Follow procedure for medication administration andnarcotics control. Utilize EMAR, scans and use 2 sources of patient ID beforeall medications and procedures.

16. Assume professional responsibility for assuringpatient care is of the highest standards possible.

17. Communicate questions about appropriateness ofmedical or nursing care through chain of command, as needed, utilizing the SBARform of communication.

18. Ensure that consents are obtained when needed, andthat all records are signed and legally complete. Ensure completion of time outprocedures as appropriate.

19. Complete incident and error forms whenappropriate.

20. Assess the patients learning needs. Educate thepatient and family.

21. Treat patients and their families with respect anddignity.

22. Practice within Nursing Standards of Practice andfollow standards of patient care & CRH Policies and Procedures. Adhere toMD orders.
