Job description

Provides wound, ostomy, continence (WOC) focused care to patients with abdominal stomas, wounds, fistulas, drains, pressure sores, dermal ulcers, and incontinence.  Responsible for various management functions (i.e., budget, supplies), educating, consulting, and researching.

Cares for patients aged neonate, pediatric, adolescent, adult, and geriatric with a variety of medical problems at various developmental levels.  Patients are from both traditional and non-traditional family structures with a variety of sociocultural and educational backgrounds.  Clients served may include in-patients, out-patients, and patients in home care, ancillary departments, and contracted facilities.

 

The Registered Nurse is responsible to assess, plan, implement and evaluate nursing care for assigned patients.  The Registered Nurse demonstrates competency in nursing practice as outlined in the Nebraska Nurse Practice Act.  The Registered Nurse must be trained to provide care for patients in their primary department.

Uses and discloses patient protected health information:  1)  Only as it applies to job functions, 2)  in amounts minimally necessary for intended purpose, and 3)  in a confidential manner.


RN Wound Ostomy + RN Occupational Health

Full-Time (40 hours a week)

Corning, Iowa

 

  1. Serves as a resource for nursing staff and other professional staff in educating patients and families in management of wound, ostomy, and continence conditions.
  2. Provides peri-operative education and stoma-site marking for patients undergoing ostomy surgery.
  3. Provides education to professional staff in topics on ostomy care, wound management, and skin care related to incontinence.
  4. Selects appropriate supplies for WOC services in conjunction with Supply Chain.
  5. Maintains expertise and keeps abreast of current knowledge in clinical practice by attending professional conferences and seminars, meeting with supply representatives, and reviewing current literature.

Provider

  1. Utilizes the nursing process to assess and prioritize patient needs.
  2. Formulates, implements and evaluates the patient’s plan of care. Adjusts plan of care in consultation with the Primary nurse as appropriate and implements plan.
  3. Uses basic communication skills to provide care to assigned patients/ families. Relays pertinent information through appropriate documentation and referral to other health care team members such as medicine, dietary and support staff.  Initiates discharge planning working in collaboration with the appropriate staff.
  4. Initiates appropriate nursing action in crisis or emergency situations.
  5. Carries out physician orders.
  6. Provides continuity of care to meet the patient’s changing needs.
  7. Records information on the patient record in accordance with charting guidelines.
  8. Utilizes time management principles and techniques.
  9. Recognizes changes in patient status based on interpretation of assessments.
  10. Recognizes examples of use of evidence/research based practice- at performance appraisal presents journal article supporting specific approach to a nursing care problem.

Teacher

  1. Uses available teaching aids to implement teaching plans of assigned patients and revises the plan based on evaluation of effectiveness.

Advocate

  1. Maintains patient confidentiality.
  2. Maintains a safe and therapeutic patient environment.
  3. Respects the rights of patients and significant others.
  4. Responsible for reporting and/or correcting potential patient safety conditions or practices.

Professional/Leader

  1. Demonstrates knowledge about the ethical, legal, professional values of nursing, guided by the American Nurses Association Code of Ethics.
  2. Demonstrates positive communication, problem solving skills.
  3. Assists the charge nurse with coordination of patient care and appropriate utilization services through use of clinical pathways.
  4. Utilizes time management principles and techniques.
  5. Effectively delegates to all levels of staff in collaboration with the charge nurse based on their scope of practice, job descriptions, abilities and patient/unit needs.
  6. Works to maintain positive team relationships with co-workers, physicians, and other members of the health care team.
  7. Supports Performance Improvement, Patient Experience and Patient Safety at the unit/organizational level.

Customer Service

  1. Receives, greets and directs patients, family members, clients, visitors and physicians in a positive and conscientious manner adhering to customer excellence standards
  2. Acknowledges internal and external customers with eye contact and a smile.
  3. Phones are answered promptly with a friendly voice using a proper greeting.
  4. Ability to prioritize, organize and effectively handle multiple tasks and interruptions in a positive manner.

Marginal Job Functions:

  1. Assumes preceptor responsibilities if trained and as needed.
  2. Participates in unit related committees, in-services and staff meetings.

BSN degree preferred. One to three years’ experience in nursing necessary in order to provide and train others in proper care of wound & ostomy patients
- After 2 years of employment in Wound Care, must pursue BSN degree (if an ADN or Diploma RN) as required due to requirements of certification.

  • Current Nebraska license as a Registered Nurse.
  • After 2 years of employment in Wound Care, must pursue certification in specialty area. (i.e. COCN, CWCN, CWOCN, CWON)
