Job Description

This Oncology Nurse Navigator position will be focused on supporting Western Maryland counties. He/she will directly support the oncologic triage and medical needs for patients diagnosed with cancer within Western Maryland. When not traveling throughout Western Maryland, he/she will be able to work remotely.

This candidate would need to be located in the Western Maryland area or willing to relocate.

In collaboration with members of the health care team, the navigator will primarily  responsible for but not limited to the following: 

  • Providing a point of clinical contact for referring providers and patients to coordinate care between referring cancer center(s) and the University of Maryland Greenebaum Comprehensive Cancer Center (UMGCCC).  

  • Facilitate transfer of care to home cancer program following delivery of care at UMGCCC.  

  • Navigate barriers to care for patients who are at risk of delay in treatment initiation resulting in poor oncologic outcomes.   

  • Offer functional knowledge of clinical trial availability. 

  • Connecting referring providers to appropriate research coordinators/principal investigators. 

Additional Responsibilities 

  • Participate in outreach to establish partnerships with clinical providers, Nurse Navigators, and administrators within the regional oncology networks. 
  • Expediate oncology consultations to UMGCC for patients with need of complicated services or second opinions. 
  • Screen patients for initial diagnosis, oncologic staging, and appropriateness of radiologic and pathologic testing.
  • Assist with the Identification of patients suitable for multi-disciplinary care
  • Assist with providing patients with IRB approved trial information. 
  • Regular attendance virtual tumor boards to coordinate care between neighboring centers to facilitate triage to adjuvant therapy services.
  • Ensure coordinated care with ancillary services for patient support during treatment including nutritional support, and social work services.
  • Leverage local and national education (NCI, ASCO, NCCN, and AONN) materials to assists patients in understanding their diagnosis, treatment options and the resources available, including educating patients about clinical research studies.
  • Facilitate transition of care to home oncology network for individuals who have completed their tertiary level medical care.
  • Work in partnership with the administrative director and solid tumor task force physician leadership to define, evaluate and implement quality measures and strategic goals for the program.
  • Train and guide other nursing and support staff engaged in clinical activities to support patient.
  • Offer family support as required, including applicable referral and palliative care refer.

Attendance and participation of at least one community screening or prevention event yearly

Qualifications

Oncology Certified Nurse (OCN®) preferred, not required.

Licensure as a Registered Nurse in the state of Maryland, or eligible to practice due to Compact state agreements outlined through the MD Board of Nursing, is required.

Bachelor’s degree required. BSN required for hires after July 1, 2008 

*Nurses on UMMC payroll by 7/1/08 must have a Bachelor’s degree or active enrollment with graduation within 5 years of promotion. Bachelor’s degree can be in any field through June 2012.  External candidates must have BSN.

Current CPR certification required.

Two years professional nursing experience required, ambulatory/outpatient experience preferred. 

Recent applicable clinical experience in a comparable setting.

Maintenance of 15 contact hours of education every year or 30 contact hours of education every two years.

Member of a professional organization.
