Speech Therapist (Prn)

Visiting Nurse Association of Southeast Missouri is looking for a Licensed Practical Nurse to join the VNA Home Health team.

ABOUT VNA HOME HEALTH: The Home Health program provides services for those needing skilled services under orders from a private physician delivered in the patient's private home. Visiting Nurse Association of Southeast Missouri is a non-profit organization.

TYPE OF WORK INVOLVED:

TRAINING: VNA provides a 30-day preceptor/orientation training for new hires. This will allow an opportunity for you to shadow our Home Health Team Members for hands-on training. It will also allow time for our Team Members to ride with you, allowing you to ask questions while making patient visits. Computer training will cover the use of the handheld used to make patient visits.

POSITION REQUIREMENTS:

Schedule initial (therapy, nutritional, etc.) assessment visit within 48 hours following the "VNA's" request for the contract services, unless otherwise authorized. Completes the initial and ongoing assessments on the specific forms/computer software provided by VNA. Works with the supervisor and the patient to develop a schedule that meets the needs of the patient, the orders of the physician, and VNA. Services are to be provided in accordance with the physician orders. The "VNA" supervisor should be notified if you are unable to provide the services as ordered. Communicate the patient's needs for discharge or transfer to the Total Care Manager prior to making contact with the physician. Coordinate with the physician to develop a plan of treatment and provide direct services to patients in accordance with the plan and the physicians orders. Review and make revisions in the plan of care based upon ongoing assessments. Submit clinical documentation & progress notes as required by regulation/policies. Assist the physician in evaluating the patient's condition.

Education:: Bachelor of Science or Masters of Science Degree in Physical Therapy having graduated from an accredited school of physical therapy that is accredited by the Commission of Accreditation for Physical Therapy Education. (unless grandfathered in under different licensing requirement prior to 1988.)

Experience: At least one year experience preferred in a health care institution in the role of PT/PTA. Home health experience preferred.
