Requisition No: 796720Agency: Department of HealthWorking Title: SENIOR LICENSED PRACTICAL NURSE - 64085223Position Number: 64085223Salary: $37,000 - $41,900.00Posting Closing Date: 03/24/2023Open CompetitiveYour Specific Responsibilities:This Senior Licensed Practical Nursing position provides clinical support to professional medical staff for clients in the Ryan White Program; Case Management as an Early Intervention Specialist (EIS); HIV Counseling, testing and linkages to care; and will provide direct patient care in the HIV Clinics in St. Johns County Health Department, under the direction of a Registered Nurse, APRN, or Licensed Physician.Clinical Support* Provides clinical support to the Ryan White Program Nurse before during and after scheduled appointments. Under the direction of the registered nurse or medical provider, is responsible for taking vitals, preparing and administering ordered vaccines, collection of laboratory specimens, (Blood urine). Responsible for preparing collected specimens for transport to state or private laboratories for analysis. Accurately inputs standard and future labs into computer and prepares computer reports as instructed. Conducts ongoing client screenings (e.g. risk, nutritional).Case Management* Works closely with new and returning to care clients with staying engaged in their care and navigating the system of care and is intended to be responsive to the immediate needs of persons living with HIV/AIDS. Follows the Supportive Case Management Processes included in the EIS Program Manual.* Assists Comprehensive Case Managers with the coordination, support, notification, and documenting, as required, of all medical, dental, mental health, or any other ordered HIV related outpatient services or procedures.* Works with EIS Program Team to send client reminder calls and ensure document preparation for any upcoming appointments.Laboratory Duties* Responsible for preparing and shipping of specimens, monitoring lab results for timeliness, and working with clinical team to address all positive labs related to the HIV Program.* Shares responsibility for updating lab processes as needed as well as the maintenance of all required logs in lab.HIV Counseling and Testing CoordinatorCoordinates HIV Counseling and Testing including scheduling, monitoring client centered anonymous and confidential HIV counseling and testing in accordance with statutory requirements and established public health policy supporting the FDOH-SJCHD walk-in testing or field testing.Clinical Supply Management for HIV ClinicsEstablish and maintain clinical services supply inventory including but not limited to weekly*
