Job description

Lutheran Life Villages is growing our Nursing Team and adding a RN/LPN Resident Care Coordinator to assist with developing, coordinating, monitoring, and collaborating all aspects of Resident Care and Nursing policies/procedures at the Village of Pine Valley Assisted Living in Fort Wayne. This Registered Nurse/Licensed Practical Nurse will assist the DON to ensure residents receive needed care and provide direct care to residents as needed.

Job Qualifications

Education/Training:

RN (Registered Nurse) preferred with degree from an accredited state approved school of nursing with a current license to practice in the State of Indiana.

LPN (Licensed Practical Nurse) with desired experience may be considered.

Experience:

Prefer a minimum of two years' experience in long term care setting with nurse supervisory or management responsibilities.

Other:

  1. Strong, verbal, organizational and interpersonal skills.
  2. Understanding of the concepts of aging and the needs and concerns of the elderly.

Essential Job Functions

  1. Coordinates, supervises and Evaluates Resident Care
  2. Functions as a member of the Nursing Management Team.
  3. Attends meetings and participates in/on committees as assigned.
  4. Receives report of residents with acute medical conditions, deteriorating status, infections, and other situations requiring close monitoring.
  5. Assists with staffing assignments and floats staff for resident coverage as assigned.
  6. Provides nursing consultation to assist in the care of specific residents; makes physician calls as necessary.
  7. Conducts goal directed resident rounds:
    1. Monitors general resident appearance, positioning, restorative measures.
    2. Monitors adherence to safety and infection control policies and procedures.
    3. Monitors effectiveness of nursing staff e.g. CNAs, QMAs, LPNs, RNs
    4. Monitors environment of sanitary and safety standards.
    5. Monitors delivery of Resident Care.
  8. Conveys results of rounds to DON/ADON and to the nursing staff; provides guidance and direction while rounding – at the elbow education/assistance.
  9. Assists other departments to organize admissions, room moves, transfers, and resident equipment needs as needed.
  10. Assists with the admission process by greeting/orienting the resident/family, transcribing physician and/or medication orders, conducting admission assessments, assisting nursing staff as necessary for a smooth transition of care.
  11. Assists with the discharge process as needed.
  12. Communicates with nursing and other departments to ensure Resident Care needs are met effectively and timely.

Quality of Care Monitoring

  1. Ensures residents care and documentation is in compliance with HIPAA, state, federal, and other rules and regulations.
  2. Assesses Resident Care, current status, significant changes, and behavior.
  3. Makes frequent rounds, visiting residents at least twice a day to monitor resident condition and responses to treatment.
  4. Assures that goals and approaches from Resident Care plan are conveyed to and implemented by the nursing staff.
  5. Observes direct care administered by staff ensuring safe, effective care is given in a timely manner as well as completion of other assigned duties.
  6. Audits documentation of medical records on a routine basis and reports findings to the DON.
  7. Monitors nursing discipline assignments to ensure completed resident assignments, plus additional duties as indicated.
  8. Observes staff adherence to resident rights policies.
  9. Assists with improving quality measures such as fall prevention, nutrition, weight loss, pain, skin/pressure areas, wound care, readmissions, etc…
  10. Participates in QAPI/PIPs as assigned; provides reports and data as required.

Development

  1. Participates in the development of efficient practices, processes, and systems within the Nursing Team.
  2. Assists in the orientation of new staff.
  3. Encourages and provides development opportunities for staff by coaching, mentoring, and identifying learning needs and educational opportunities for staff to grow personally and professionally and provide or arrange training accordingly.
  4. Researches and identifies new nursing practices and standard of care; identifies and seeks out training opportunities to learn personally and professionally.
  5. Knows and supports facility and nursing policies and procedures and understands and abides by the American Nursing Association nursing Code of Ethics.
  6. Maintains professional competence by keeping abreast of developments in the field of geriatrics, long-term care, and professional issues related to the long-term care setting.
  7. Identifies and attends continuing education offerings which enhance the clinical care role in the long-term care setting.
  8. Attends and participates in facility meetings, and all mandatory in-services.
  9. Observes care being given including physical, psychological and rehabilitative aspects.
  10. Identifies learning needs of assigned staff; may implement mini-in-service in Resident Care area; contributes to topics for regular in-service programs.
  11. Instructs nursing staff on expectations relating to staff performance, Resident Care, and environmental monitoring.
  12. Communicates with Nursing Team regarding training needs, resident needs, and supply/equipment needs.
  13. Ensures assigned nursing staff are current on certifications (TB & CPR).

Oversight

  1. Oversees the daily functions of nursing staff and ensures effective care is given in a timely manner.
  2. Oversees Resident Care delivery to include but not limited to physician orders, assessments, appointments, labs, treatment plan changes, and behaviors.
  3. Actively makes frequent rounds, visiting residents to monitor condition and responses to treatment; observes care being given including physical, psychological and rehabilitative aspects.
  4. Practices good body mechanics and monitors practices of nursing staff encouraging use of available lifting equipment, devices, and labor saving practices.
  5. Oversees nursing processes to include but not limited to care delivery and responses to treatment, med pass, treatment plans, reporting of acute changes, admissions, readmissions, discharges, and care planning.
  6. Assures infection control, standard precautions, and medication practices are maintained. May serve as the Infection Preventionist if certified.
  7. May serve as a wound care nurse if certified.
  8. May serve as the Restorative Nursing Program coordinator if assigned.
  9. Reviews resident records and documentation to ensure programs are in place according to regulations and LLV requirements.
  10. Assists with instructing nursing staff on expectations relating to staff performance, Resident Care, and environmental monitoring.
  11. Observes care being given including physical, psychological and rehabilitative aspects in line with resident rights.
  12. Assists in emergency situations as needed.

Collaborates

  1. Attends and actively participates in IDT Team Meetings; provides updates on resident matters and outcomes as requested by DON.
  2. Exhibits listening skills in sensitive situations; has the ability to provide guidance and problem solve with assigned nursing team, family members, and residents.
  3. Determines appropriate flow of work with nursing team for the most efficient outcomes.
  4. Creates a supportive and compassionate environment to promote teamwork and positivity.
  5. Encourages and promotes personal and professional development, nursing strategy development, and encourages all nursing staff bring forth new ideas and solutions and to adapt and evolve within their roles.
  6. Share information and knowledge including but not limited to staff concerns, resident conditions, complaints, environmental concerns promptly with DON.
  7. Encourage suggestions and ideas and share information and knowledge with staff members to provide improved work flow or improved quality of life for residents.
  8. Enable team growth by allocating time and resources as appropriate for training and development.
  9. Allow roles and responsibilities to evolve and fluctuate to empower staff to progress personally and professionally in their nursing roles.
  10. Provides suggestions and alternatives to improve operation of the nursing department and provision of Resident Care.

           

Adheres to and enforces safety measures.

  1. Practices good body mechanics and monitors practices of assigned staff,      encouraging use of available lifting equipment, devices and labor saving practices.
  2. Assures infection control, standard precautions, and medication practices      are maintained.
  3. Completes accident/incident, medication incident and infection control reports according to facility policies.
  4. Maintains current knowledge of scope of practice for self and others recognizing legal implications when making nursing judgments.
  5. Uses and directs to us protective devices when performing tasks according      to OSHA, infection control and standard precautions regulations.
  6. Reports all hazardous conditions and defective equipment.
  7. Follow and instructs others to follow fire/tornado/emergency policies and procedures.
  8. Keeps work area free of hazards and monitors employee ability to maintain a hazard free work area.
  9. Follows and instructs others to follow safety precautions when using equipment and supplies.

 

OTHER RESPONSIBILITIES:

  1. Monitors and makes recommendations regarding safe and effective nursing care practices.
  2. Keeps facility manuals in visible and accessible location for use.
  3. Adheres to and monitors efficient and courteous telephone use.
  4. Conducts special projects as requested by the Director of Nursing.
  5. Any other duties as assigned.
