RN ACF Coordinator/ Case Manager Schedule
Monday - Friday 7am - 3:30pm with on call rotation
Positions Summary
The RN ACF Coordinator/ Case Manager is responsible for ensuring the overall health care of the residents on the unit with appropriate clinical interventions. Initial and periodic evaluations of the residents and the development of the Individualized Service Pan (ISP) for each resident that identifies and evaluates the resident’s needs, interests, and strengths and the capability of the facility to meet those needs. Provide person centered care in accordance with physician orders, facility policies, and federal/state regulations.
Essential Job Requirements
Education: Two (2) year Associate's Degree
License: Active New York State RN license
Qualifications: BLS and/or CPR certification.
Must be 18 years of age. Ability to read, write and speak the English language
Experience: Three (3) years in ALR/SNF facility preferred, knowledge of Department of Health regulations. Excellent
clinical skills, ability to work well with medical providers, family members, and staff
