SalaryUnder the direction of the AVP of Managed Care and the Sr. Manager, Risk Adjustment Programs (RAP) for the MSO (Altura), the Coordinator supports and promotes efficient communications within the RAP team and with external partners including Health Plans, AltaMed Clinics and IPA Physician Offices. Monitors and tracks incoming communications (via email, mail, phone), logs the information in the appropriate location and notifies the individuals or teams as needed; tracking deliverables for follow up and timely response. The RAP Coordinator will support all communications, documentation, training and reporting training, monitoring and reporting needs related to risk assessment, documentation, audit and data capture activity related to HCC, and Medicare 5-Star programs and the pay for performance incentive programs, other incentive and/or quality related programs.
Responsibilities1. Monitors and tracks incoming communications (email, mail, phone). Stores documents in Microsoft Teams / SharePoint as appropriate. Distributes to the appropriate internal person or team.
2. Supports RAP department in maintenance of all Health Plan communications related to RAP, HCC, RAF, or Medicare 5-Star measures.
3. Scan documents received via mail logs in to appropriate location(s).
4. Track and report documentation, assessments form completion and other activity necessary to support the capture and appropriate coding as requested.
5. Collaborates with internal departments to distribute, track, and monitor responses or deliverables.
6. Schedule group and individual trainings as necessary to support departmental and organizational objectives.
7. Coordinate appointments between AltaMed corporate staff and clinics and IPA physicians for HCC/RAF and 5 Star and P4P program related trainings.
8. Routinely provide performance reports to providers, administrators and others as necessary to improve performance.
9. Build and maintain relationships with key staff in priority sites to improve on existing HCC/RAF policies in place.
10. Remain current with HCC/RAF and risk adjustment trends and 5 Star and Pay for Performance programs and related industry changes as appropriate.
11. Attend staff meetings or onsite HCC/RAF and risk adjustments and 5 Star and Pay for Performance and other events as needed.
12. Maintains department Policy and Procedure documents.
13. Assist the Incentive programs coder to meet departmental goals.
14. Performs other related duties as assigned.
Qualifications
1. Minimum high school diploma. B.S./B.A. degree in healthcare or business administration or a related field of study preferred.
2. Three years minimum experience in Customer or Provider Service required.
3. One-year experience in a health care / managed care setting preferred.
