Description
The Psychiatric Medical Director is the highest-ranking psychiatric physician leader within the Custody Healthcare Services Adult Jail Facilities (Main Jail /Elmwood) responsible for planning, organizing, directing and evaluating all psychiatric activity within the two adult jail facilities and ensures compassionate, safe, effective, timely, efficient and equitable patient-centered care in conjunction with other health care discipline leaders, and in coordination with deliverables under Federal Consent Decree(s) for mental health.

This position is under the direct supervision of the Director, Custody Health Services and receives clinical oversight from the Enterprise Medical Staff of Santa Clara Valley Healthcare (SCVH) insofar as it pertains to standards of psychiatric practice, system-wide goals and objectives, and compliance with medical staff by-laws (Professional credentialing, privileging, performance evaluation, peer review, provider performance monitoring and feedback, etc.). This position assumes comprehensive 24-hour and 7-day (24/7) accountability for the psychiatric operations and delivery of all patient care psychiatric services; assigns work and specific cases; consults on critical psychiatric cases, and monitors patient-inmate course in care, continuity of care, chronic mental health care and psychotropic medications to include Penal Code (PC) 2603.

The Psychiatric Medical Director ensures patients have ready access to mental health services based on their need and that the individual level of functioning of seriously mentally disordered inmate-patients is optimized so their care may be maintained in the least restrictive environment. This position may perform psychiatric care of patients within the facilities, as necessary.

Typical Tasks
  • Administers, evaluates, and approves the treatment, education, and research services;
  • Oversees a large clinical staff up to and including psychiatrists, psychologists, social workers, therapists, and psychiatric technicians;
  • Coordinates facility treatment functions with community health organizations by meeting with local government officials, agency representatives, community planning and advisory groups, volunteer groups, educational institutions, and others;
  • Implements Federal and State laws and departmental treatment policies;
  • Assures that treatment services are developed within the context of an overall planned effort with explicit goals and objectives to meet the Federal Consent Decrees, and consistent with the mission of the Custody Health Services Department;
  • Assures that overall planning is relevant to the needs of the patients and consistent with community needs and the Custody Health delivery system within the resources available;
  • Assures that there is a full range of appropriate support services which provide quality care and programming to the patients;
  • Resolves problems and establishes policies pertaining to treatment services in conjunction with other CHS leaders;
  • Insures that the treatment services achieve and maintain compliance with Title 15 and state and federal regulations;
  • Provides consultation on difficult cases within the Jails to include with Federal Monitors and outside State hospital or developmental center;
  • Consults with staff members on unusual or difficult cases or other treatment problems.

Employment Standards
Must have leadership experience similar to a Psychiatric Director or Chief Psychiatric Officer with an acute psychiatric healthcare system, and minimum four years of general psychiatric clinic experience as a practicing physician. Experience working in a county jail or state prison psychiatric services unit is highly desirable.

License/Certificate Requirements:

  • Possession of a valid license to practice medicine in the State of California.
  • Possession of a valid certificate in psychiatry issued by the American Board of Psychiatry and Neurology or the American Osteopathic Board of Neurology and Psychiatry.
  • Possession of a certificate of graduation from an ACGME accredited psychiatry residency program.
  • Possession and maintenance of a State and/or Federal narcotic license for administration of narcotics to inmates and patients.


Thorough knowledge of:

  • Current principles and practices of medicine.
  • Accepted psychiatric practices in the use of advanced psychopharmacological skills and psychotropic medication and stabilization practices.
  • Accepted psychiatric practices related to the nexus between substance use disorders and other mental illnesses.
  • Accepted psychiatric practices related to trauma and trauma-informed care.
  • Accepted psychiatric practices related mental health patients and complex chronic care disease and management.
Working knowledge of:
  • Principles of leadership, supervision and training.

Ability to:
  • Provide psychiatric direction to and supervise subordinate staff.
  • Perform psychiatric diagnosis and treatment.
  • Evaluate and review the work of psychiatric practitioners for appropriate diagnosis, treatment and chart documentation.
  • Plan and prepare protocols and standardized procedures.
  • Coordinate and evaluate the work of contract and staff psychiatrists.
  • Participate in the analysis, evaluation and planning of jails mental health programs and continuum of care.
  • Prepare clear and concise written and oral reports.
  • Analyze situations accurately and adopt an effective course of action.
  • Conduct in-service training programs.
  • Learn to input, access and analyze data using a computer terminal.
  • Learn to use automated formulary, order entry and Electronic Medical Record (EMR) software.
