Functions to administer care for adult clients of all ages in their place of residence, coordinate care with the interdisciplinary team, patient/family and referring agency, and other staff

CNA's are NOT eligible for this job position.
Graduate of an accredited Diploma or Associate School of Nursing
Current Texas State license, current Texas Driver’s License.
Two years’ experience in a clinical care setting is preferred.
Nursing skills as defined by generally accepted standards of practice. Good interpersonal skills.
Reliable transportation and valid and current auto liability insurance
Works in patients' homes in various conditions; proof of current CPR, and Hepatitis consent or declination; possible exposure to blood and bodily fluids and infectious diseases; ability to work a flexible schedule; ability to travel locally; some exposure to unpleasant weather; PRN emergency call.
Standing and walking are required, with the ability to lift up to 50 lbs. and move patients. Requires working under some stressful conditions to meet deadlines and patient needs, and to make quick decisions and resource acquisition; meet patient/family individualized psychosocial needs. Requires hand-eye coordination and manual dexterity.
Implement/Document the plan of care to ensure quality and continuity of care.
Provides care for all clients within scope.
Reports symptoms, reactions, and progress of patient to the clinical supervisor and/or charge nurse to ensure appropriate action has been taken.
Provide care utilizing infection control measures that protect both the staff and the patient (OSHA).
Assure continuity of quality patient care delivered with appropriate documentation.
· Monitor assigned cases to ensure compliance with the requirements of the third-party payor.
· Demonstrate commitment, professional growth, and competency.
· Promote Agency philosophy and administrative policies.
· Perform on-call responsibilities and provide on-call service to patients/families as assigned.
· Effective communication with patient/family, team members, and other health care professionals.
