Job Description:

Job Summary: The Program Manager of Transitional and Post-Acute Care Services (T-PAC) plays a vital role in achieving positive patient outcomes in the market including but not limited to: improving the length of stay (LOS), increasing the efficiency of PAC provider's delivery of care, increasing discharges to preferred PAC Providers, monitoring of readmission trends, tracking quality of care measures across all preferred PAC Providers, and educating hospital and PAC leaders on strategies to improve performance. The Program Manager works closely with a variety of stakeholders, coordinating the activities of T-PAC across the enterprise. This position is responsible for advancing strategies to optimize the transition of patients across the continuum of care; working with post-acute facilities and services to develop partnerships, collaborate on clinical processes, establish, and evaluate metrics.

Essential Job Functions & Accountabilities:

  1. Assists leaders in communicating and aligning work priorities with the organizational vision, mission, values, and service standards.

  2. Establishes, meets and continuously monitors the goals and objectives while maintaining alignment with the strategic goals and objectives for JPS Health Network.

  3. Responsible for developing a network of preferred transitional post-acute care (T-PAC) providers for the enterprise.

  4. Collaborates with post-acute clinical staff and leadership to ensure attainment and alignment of resources (contracts, EMR access, onsite access, etc.) and post-acute providers.

  5. Responsible for maintaining positive relationships with PAC providers and hospital staff to support the patient-centric model in a highly collaborative PAC network.

  6. Provides regular onsite reviews of PAC provider operations to ensure quality patient care and staff support and communicates results to hospital and PAC leadership.

  7. Executes department goals by leading initiatives, meeting deadlines, and coordinating resources between hospitals and preferred PAC providers to ensure efficient and effective operations.

  8. Gathers and monitors relevant data regarding the PAC provider community and the changing dynamics within the market.

  9. Monitors reporting database of PAC provider metrics (e.g. readmission rates, ALOS, publicly reported quality metrics, patient experience, etc.).

  10. Identifies challenging situations early and ensures appropriate action is taken to mitigate escalation, works to resolve PAC provider complaints, and leads PAC network issue resolution.

  11. Educates hospital and PAC leaders on strategies to improve performance.

Qualifications:


    Required Qualifications:
  • Bachelor's Degree in therapy, nursing, social work, business, or other healthcare management-related field required
  • 5 years of relevant experience in a healthcare setting.

  • Preferred Qualifications:
  • Master's degree in Health Services Administration, Business Administration, Nursing or a related field of study from an accredited college or university.
  • Leadership experience in post-acute services.
  • Experience in a complex health system.
  • Current RN licensure by the Board of Nurse Examiners for the State of Texas or proof of reciprocity of licensure between the State of Texas and another state.
  • Licensed Master Social Work (LMSW) through the Texas State Board of Social Work Examiners.
  • Current licensure by Texas Board of Physical and Occupational Therapy Examiners.
