Primary Function:
Develops and implements physical therapy services for patients; works as part of a multidisciplinary team providing quality healthcare to all patients in accordance with the principle of ethics and code of professional conduct.
Duties and Responsibilities:
Evaluates a patient's condition and needs; Conducts assessments; Reviews the medical history to develop a comprehensive profile.
Develops individualized physical therapy plans for patients, laying out the types of activities and specific goals to be accomplished and outcomes to achieve.
Demonstrates exercises and techniques and works with patients to ensure their understanding and ability to perform the treatments assigned.
Works with, educates and maintains communication with a patient's family as needed.
Makes recommendations about special equipment and instructs patients on how to use that equipment.
Assesses and records patients' activities and progress for patient evaluations; Documents encounters in the Electronic Medical Record system; Codes progress notes for billing purposes.
Functions as part of a multidisciplinary team; provides other healthcare providers with reports to ensure coordination of care; Participates in and contributes to Quality Improvement and Quality Assurance activities and functions as needed.
Provides education.
Provides clinical direction to the Physical Therapist Assistant and other support staff.
Adhere to all CHCB's clinical policies and procedures, including infection control principles and confidentiality.
Assist with triage of administrative questions as appropriate.
Comply with all the guidelines in accordance with all regulatory agencies governing health care delivery.
Develops and reviews policies and procedures.
Attend meetings as required.
Performs special assignments as requested by the Chief Executive Officer and/or Chief Medical Officer.
Direct, instruct and evaluate medical students assigned to CHCB.
Physical Demands: Requires period of sitting, standing, walking, bending and stooping. Significant job stress may be associated with the position. Evening work may be required.
Minimum Qualifications: Master's Degree in Physical Therapy. Possession of a license and current registration to practice Physical Therapy in New York State. Computer literacy. Experience using an Electronic Medical Record System. Proficiency in MS Office.
Preferred Qualifications: Doctorate Degree. Specialty focus certifications earned through the American Physical Therapy Association. Basic and Advanced Cardiac Life Support Certification. Professional Membership Affiliations.Experience with eClinical Works.
