Job Summary
The Medication Reconciliation Pharmacy Technician is responsible for working with newly admitted patients to obtain and document an accurate home medication list in an effort to help avoid medication duplications, omissions, and errors.

Essential Functions
- Obtains accurate medication histories for patients admitted to the hospital
- Conducts patient and/or caregiver interviews to obtain current medication regimens including prescription and over-the-counter medications, as well as vitamin and herbal supplements for dosing, frequency, and indication for use
- Consults with patient's pharmacy or provider(s) to investigate and verify medication discrepancies when necessary
- Provides limited operational support to the pharmacy when needed

Qualifications
Minimum Education
High school diploma or equivalent required
Required Skills
Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.
Required Licenses/Certifications
- Current certification with the Pharmacy Technician Certification Board (PTCB)
- Registration with the North Carolina Board of Pharmacy (NCBOP) within 30 days of hire
2+ Years of retail pharmacy technician experience
