The Nursing Assistant will provide personal care and nursing care to day participants under the direction of the Adult Day Health Program Nurse and will assist in developing, implementing, and evaluating recreational programs to meet the physical, social, intellectual, emotional and cultural needs of day participants.Core Competencies:
- Recognize and report any change in condition of the participant to Adult Day Health Nurse.
- Maintain confidentiality of participant information.
- Participate in educational programs within HSL, to maintain and update work-related knowledge and skills.
- Demonstrate a strong commitment to the philosophy and mission of the Great Days for Seniors program and recognize participants’ dignity and choice in all aspects of daily life. Work with team to meet all needs of the participant to actively participate in activities, work and the social environment. Strive to make every encounter with a participant into a positive and meaningful experience, while providing safe and efficient quality of care.
Position Responsibilities:
- In collaboration with all ADH staff, assist participants with Activities of Daily Living as needed, including transport, transfer, mobility, mealtime assistance, personal care and toileting needs of participants.
- Plan and implement day program recreational activities in conjunction with the Activities Coordinator.
- Assist in assessing progress of participants.
- Assist in ordering and maintaining recreation supplies.
- Assist in planning and organizing special events.
- Participate in all program transition times, including daily arrival and departure, participant transporting, meals, activity set-up and clean-up.
- Impart pertinent recreation information to day participants.
- Set up, distribute and clean up after morning and afternoon snacks.
- Obtain blood pressure readings, pulses and weights of participants and document appropriately.
- Participate in participant care planning activities and Adult Day Health Department meetings.
- Complete daily ADL flow sheets for all participants and other nursing or activities documentation as needed.
- Help out with clerical tasks as needed: data entry, copying, filing, etc.
- Perform other related duties as required.
- High School Diploma or equivalent required.
- Must have at least 1-year experience working with the elderly and an interest in running activity groups.
- Nurse Assistant or Home Health Aide Certification required.
- Experience in or interest in leading activity groups required.
- Multilingual English/Cantonese/Mandarin or Russian speaking required.
