Patient Services Coordinator


We are on the lookout for a passionate and motivated Patient Services Coordinator (PSC) , who is bilingual in English & Spanish to join our team! As a PSC with us, you will be an integral part of the care team, operating alongside a Primary Care Provider, Medical Assistant, and a Population Health Coach. We redesigned our care teams with patient access and care in mind. By moving the PSC to sit with the Care Team instead of the front office reception, we are able to give patients direct access to their care team, continuing to strengthen the relationship that each member of the care team has with their patient.


As a PSC with Winters Healthcare, you will be the primary recipient of phone calls and emails from patients on your Care Team's panel. As the keeper to the care team's schedule, the PSC will monitor the daily clinic schedule as well as assist patients by scheduling and confirming appointments, documenting in the electronic health record, verify patient demographics, "scrub" patient charts, determine insurance eligibility, collect payment, and refer patients to other resources as needed. The PSC will also take on a partnership role with the Population Health Coach, by reviewing and monitoring patient data to call and schedule patients for preventative care when due.

Responsibilities:

  • Communicate, collaborate, and work in a team-oriented environment supporting our model of care
  • Greet patients, answer phones, schedule and confirm appointments, and monitor incoming correspondence and requests
  • Review patient data to actively call and engage patients to schedule an initial, follow-up, and/or preventative visit with the care team
  • Manage care team's clinic schedule, "scrub" charts, determine insurance coverage and eligibility for various programs

Qualifications:

  • Bilingual fluency in English/Spanish is required (ability to read, write, and speak)
  • Significant background providing outstanding customer service
  • Strong computer skills; knowledge of eClinical Works (ECW) is preferred
  • Experience in a medical setting is preferred and experience in a Federally Qualified Health Center is a plus!

Hours:

  • This position will be a regular, full-time position at 40 hours per week. A minimum of one evening shift per week is required. You will have a pretty set schedule with 3 or 4 days scheduled from 8am - 5pm, and 1 or 2 days scheduled from 11am - 8pm.

Compensation and Benefits:

  • Hourly rate range at $17 to $20.50/ hour and depends on experience

Patient Services Coordinator

Reports to: Front Office Manager

FLSA Status: Nonexempt, hourly

Summary of Duties:

The Patient Services Coordinator (PSC) is an integral part of the care team, operating alongside a Primary Care Provider, Medical Assistant, and a Population Health Coach. We redesigned our care teams with patient access and care in mind. The PSC is the primary recipient of phone calls and emails from patients on the Care Team's panel. The PSC is the keeper and monitor of the daily clinic schedule, and assists patients by scheduling and confirming appointments, documenting in the electronic health record, verifying patient demographics, "scrubbing" patient charts, determining insurance eligibility, collecting payment, and referring patients to other resources as needed. The PSC also partners with the Population Health Coach by reviewing and monitoring patient data to call and schedule patients for preventative care when due.

Essential Functions:

  • Communicate, collaborate, and work in a team-oriented environment supporting Relationship Based Care
  • Monitor daily clinic schedule, adjust appointment times and communicate with patients as needed, ensuring a smooth clinic flow
  • Answer patient calls as they come in to direct care team phone line, answering questions, and attending to patient's reason for the call
  • Monitor incoming patient correspondence and requests, and respond or route correspondence as indicated
  • Work with the medical call center closely for new patient assignments and engagement activities
  • Work closely with Population Health Coach to identify/review data and schedule preventive care
  • Call patient for all missed appointments to ensure they are rescheduled for care services
  • Appropriately schedule/reschedule appointments as requested, following appointment guidelines
  • Verify patient demographics, insurance coverage and determine eligibility for various programs; offering SFS to non-insured patients, providing qualifications for SFS, and determine eligibility
  • Responsible for verifying if patient had a recent visit to the ER when scheduling appointment, and use the various hospital access portals (Dignity, Sutter, Northbay) to print records from ER visit.
  • Message RN Care Coordinator for pre-visit review of medications and any additional work up needed prior to follow-up visit.
  • Monitor telephone encounters and voicemail messages, addressing within one business day.
  • When receiving a patient complaint, remain calm, acknowledge the patient's concern, and follow up with the team to debrief and rectify issue.
  • Work closely with Front Office Supervisor and Medical Assistant Supervisor on new and updated workflows, trainings, resources, and support
  • Participate in educational activities and attend staff meetings
  • Provide occasional coverage for the Medical office and/or call center as needed
  • Support performance and quality improvement efforts, participating in related activities as required.
  • Other duties as assigned

Minimum Requirements

Education and Work Experience

  • High school diploma or equivalent,
  • Minimum of one year in a medical setting with medical front office experience or medical call center experience,
  • Significant background providing outstanding customer service
  • CPR certification (or within 60 days of hire)

Knowledge, Skills and Abilities:

  • Fluency (the ability to read, write and speak) in English and Spanish
  • Well-organized and detail-oriented with the ability to establish and maintain working relationships with patients, employees and the public
  • Experience operating standard office equipment
  • Requires effective oral and written communication skills, excellent interpersonal skills, & computer literacy.
  • Emotional intelligence and critical thinking skills
  • Ability and comfort building relationships with coworkers and patients
  • Ability to remain calm under pressure, whether internal or external pressure
  • Ability to engage with patients to schedule visits, or to understand barriers to attending visits
  • Demonstrated team experience and proven successes in implementation of team goals
  • Proficient in computer programs such as Microsoft Office: Word, Excel, Outlook
  • A passion and dedication for serving the healthcare needs of our clients
  • Previous experience with FQHC, low income, and/or medically underserved populations preferred

Other Requirements:

  • Authorization and consent for Winters Healthcare to investigate candidate's background with a consumer report for employment purposes, to evaluate candidate/employee for employment, reassignment, or retention as an employee
  • As a condition of employment all current and newly hired Winters Healthcare employees must provide proof of being fully vaccinated against COVID-19.

Operating hours:

  • Monday, Tuesday and Thursday: 8:00 am - 8:00pm; Wednesday and Friday: 8:00 am - 5:00pm.

Working Hours:

  • This position will be a regular, full-time position at 40 hours per week. A minimum of one evening shift per week is required (11am - 8pm), with other days scheduled from 8am - 5pm.

While performing the duties and tasks of this job, Employee is regularly required to stand; walk; work on irregular surfaces; reach with hands and arms; use hands to finger, handle or feel objects, tools, or controls; talk or hear; and taste or smell. Employee is occasionally required to sit and stoop, kneel, crouch, bend, squat, twist or crawl. This job requires physical effort and the ability to place or retrieve items at below waist level may be required.

Employee must frequently lift and/or move up to 10 pounds and occasionally lift and/or move up to 25 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision and depth perception, and the ability to adjust focus. Employee occasionally may be required to transfer patients to/from a wheelchair. The physical demands described here are representative of those that must be met by Employee to successfully perform the essential functions of this job. Environmental/ Working Conditions:

The environment is a standard medical office environment. Regular office hours are Monday through Friday. Evenings and overtime may be required. Office hours may be changed as business needs dictate.
