Pathways Health Partners is an Accountable Care Organization (ACO) on the forefront of helping independent providers make the transition to Value-Based Care. Pathways operates a Medicare REACH ACO, a Medicare Advantage MSO, a Commercial MSO responsible for approximately 16,000 patients throughout North-West/Central Florida. Our service area is The Villages, down to St. Petersburg, across to Mount Dora, and areas in between. Pathways also operates a robust hospital medicine group, manages several of its affiliated medical practices, and an insurance agency.
Job Summary - Seeking for Marion, Citrus, and Pinellas Counties
The Patient Engagement Specialist enables patient care through care coordination and team-based care. The Patient Engagement Specialist will assess, plan, implement, coordinate, monitor, evaluate healthcare options and services with the goal of increasing the likelihood of improvements to the health status of identified populations and specific patients across the system. In addition, Patient Engagement Specialist is responsible for applying the appropriate Patient Engagement processes to support eh patient’s needs, promote wellness screenings, and minimize potential health risks. A reduction in patient utilization of the emergency department would be an indicator of success.
Role and Responsibilities
Position Competencies:
- Triage reason for ER visit
- Assess patient(s) for healthcare access including provider appointments and pharmacy needs
- Proper driving utilizing safe driver techniques, avoid distracted driving, maintain control of vehicle, and obey traffic laws.
- Respect and protect patient and facility confidentiality by keeping all sensitive and patient information private
Position Responsibilities:
Specific:
- Engage patient(s) the day of an Emergency Room (ER) visit
- Work in Tandem with the Pathways care team to determine if a visit with Primary Care Physician (PCP is needed)
- Educate Patient on calling Primary Care Physician as a possible first option
- Deliver Pathways magnet to patient
- Education patient on the emergency room utilization (aka “Zones”)
- Patient Transport (if/when transporting)
- Establishes and maintains communication with patients and coworkers to improve the quality of service and patient satisfaction
- Comforts patients and their families by offering support and encouragement; being punctual and attentive to procedure schedules; demonstrating caring and cultural sensitivity
- Transports medication, equipment, and supplies by picking up requisitioned orders; delivering them to assigned patient care areas including the patient home
- Maintains the transportation equipment to provide a safe, secure, and healthy work and patient care environment following standards and procedures
- Follows all protocols and procedures to ensure the safe deliver of patients, their possessions, supplies and specimens

Qualifications and Education Requirements
- High School Diploma or GED required
- Maintain an Acceptable Florida Driver’s License
- No points on Florida Driver’s Record, no accidents or moving violations
- Medical Assistant, LPN, Paramedic preferred. Florida Driver License (no points, accidents, or moving violations and maintains this rating during employment). CPR/AED/First Aid or BLS certification at time of higher or within 30 days. EMT or equivalent certification highly desirable.
