Job Description
This position is eligible for American Rescue Plan Act (ARPA) funds which includes a one-time $750.00 sign-on bonus payable after the completion of 90 days of employment as well as a monthly stipend of up to $694.50 which is based upon the hours worked in the previous month. These funds are guaranteed through June 30, 2023.

Job Summary
This position is hybrid so he/she has the flexibility to work part in person and part virtual. He/she will take responsibility for their assigned clients by providing outreach family counseling. This person will utilize a variety of treatment interventions while working with a wide caseload, working as a team with nonclinical caseworkers. The nature of the program and workflow will require this person to provide continued follow-through and supervision beyond the initial crisis stage, helping families cope with their immediate problems as well as finding longer-term solutions.
Due to the nature of the workflow and level of responsibility this individual maintains a flexible schedule, which would include nights and weekends, alternating on-call duties are also assigned.

Responsibilities and Duties
- Provide guidance and case coordination of all open cases.
- Work as a team member with EFSS Caseworkers, when clinically indicated, and supervisor to monitor services and ensure advocacy for families.
- Utilize home-based strength model to provide a wide range of assistance and resources to families in the process of reunification or to avoid separation.
- Treat psychological and emotional issues that affect parenting.
- Implement treatment modalities that address chronic and pathological issues that impact family functioning.
- Provide community and home based therapy to secure a safe and independent level of functioning.
- Contact the family in crisis as soon as possible following the reported crisis, and conduct an initial clinical assessment of the situation.
- Develop, implement and monitor an ongoing case plan for each family served; coordinate all service components both within the agency and in the community.
- Work with each family to recognize and sustain gains that have been made.
- Make referrals to other service providers when necessary.
- Schedule and coordinate case conferences and meetings with service providers.
- Available for emergency on-call response to crisis situations.
- Support family reunification.
- Coordinate services with schools, police, health care provider, court systems, and other service providers.
- Handle day-to-day community relations regarding the interactions of the program and its clients in the community.
- Ensures that the department’s scope of service strives to meet the needs and expectations of internal and external customers.
- Ensures all clients receive quality services, which comply with the standards set by the Agency, licensing agents, funding sources, and other relevant organization.
- Ensures all clients receive goal-oriented assessments and treatment/service plans within established time frames.
- Ensures client intake and termination dates and demographic information is communicated for the timely completion of billing.
Qualifications and Skills
- Master's degree in Social Work or related field required; licensure (LCSW, LMHC, LMFT) preferred
- Valid driver's license required
