We are currently seeking a dedicated and experienced Occupational Therapist (OT) specializing in Home Health to join our team. As an OT in Home Health, you will play a crucial role in providing therapy and rehabilitation services to adult patients in the comfort of their own homes.

Responsibilities:

• Conduct comprehensive assessments of patients' functional abilities and limitations in their home environment.
• Develop personalized treatment plans to address patients' specific needs and goals.
• Provide hands-on therapy interventions to improve patients' mobility, strength, coordination, and daily living skills.
• Educate patients and their families on adaptive strategies and equipment to maximize independence and safety.
• Monitor and document patients' progress, adjust treatment plans as needed, and communicate effectively with the healthcare team.
• Collaborate with other healthcare professionals, such as physicians, nurses, and social workers, to ensure coordinated and holistic patient care.
• Adhere to professional and ethical guidelines, as well as legal requirements, in the provision of occupational therapy services.
• Complete all necessary documentation, including evaluations, progress notes, and discharge summaries, accurately and in a timely manner.

Qualifications:

• Active state license as an Occupational Therapist.
• Proficiency in using Home Care Home Base and Oasis software.
• Minimum of 2 years of previous experience in adult Home Health.
• Strong clinical skills in evaluating and treating patients with various diagnoses and conditions in a home setting.
• Ability to work independently and efficiently manage a caseload.
• Willingness to consider temp-to-hire opportunities.
• Valid driver's license and reliable transportation to travel between patient homes.
• Mileage reimbursement is provided based on IRS standards.
• Ability to meet productivity targets of 30 points.
• No on-call responsibilities, unless explicitly stated upfront.
• OT preapproved by branch.
