Position Overview
This Nurse Liaison (RN) position is primarily responsible for assisting the Referrals and Admissions Department in admitting patients onto agency services including hospice, palliative care, Staywell and other programs offered by Hospice of the Piedmont. The Nurse Liaison participates in admissions, pre-assessments, and other related visits across various settings including the home, inpatient units, hospitals, long-term care facilities, or all other settings as necessary. Nurse Liaison shall assure appropriateness for services, verify orders, and communicate with other members of the interdisciplinary team to develop a plan of care.
Essential Duties
- Assists with referral intake process by providing program information to prospective patients, families, and referral sources. Provides information, collects information, and develops the referral according to agency procedure. May be involved in the election statement signing.
- Develops and maintains a hospice presence in hospitals, long-term care facilities, and other targeted referral sources by coordinating with providers, case managers, and other staff that have direct involvement in the referrals and admissions process.
- Does initial assessments in the home, facility, hospital, or any other settings as needed to evaluate physical, environmental/safety needs of patient to explain services and to develop an initial plan of care in collaboration with patient, significant others, and interdisciplinary team members in order to assure satisfactory patient outcome.
- Evaluates patient’s appropriateness and eligibility for program services. Communicates with primary physician regarding assessment findings and initiation of treatment plan. Nurse will communicate with other members of the interdisciplinary team in order to introduce the patient.
- Accurately and timely documents communication with patient/family and other medical personnel involved in the case according to the policies of Hospice of the Piedmont. Documents same in acute care facility’s chart according to procedures arranged with that facility.
- Assesses physical, environmental and safety needs of patient/family and establishes initial plan of care. Reviews all medications patient is taking, compiles list for medical record, and checks for drug/drug interactions. Provides initial instruction of patient/caregiver regarding safe and effective management of the patient in the home and program requirements.
- Acts as a liaison and advocate for hospice patients admitted to the hospital, ensuring that the hospice plan of care is adhered to during the in-patient time. Also, will help determine if inpatient admission of hospice patient is related to terminal illness.
- Documents according to agency policy and procedures; documentation is completed accurately and timely.
- Orders equipment, supplies and other services as necessary for the patient’s plan of care.
- Complies with organizational nursing standards.
- Maintains confidentiality of patients and their families, co-workers, volunteers, and the organization at all times.
- Adheres to the organization’s Standards of Conduct and HIPAA regulations.
- Reports any potential misconduct or violation of the organization’s Standards of Conduct, HIPAA regulations, or federal or state laws and regulations.
Non-Essential Duties
- Participates in Referrals and Admission Department on-call rotation.
- Assists with business development activities as schedule permits.
- Makes home nursing visits prn to assist staff nurses in following the patient’s plan of care.
- Other duties as assigned to promote the efficiency and effectiveness of the organization
