The Inpatient Care Coordinator, in collaboration with the attending practitioner, the family and the multidisciplinary health care team, is responsible for utilizing Case Management, Care Coordination and utilization management activities to ensure that hospitalized patients receive quality, cost-effective patient care in the most appropriate setting. The Inpatient Care Coordinator facilitates the development of a comprehensive discharge plan that is individualized to the patients' needs and utilizes clinical expertise and critical thinking to continually monitor and revise the plan based on the patient's clinical status and desired outcomes. The Inpatient Care Coordinator is responsible for ensuring the patient's safe and timely transition to alternate levels of care by communicating and collaborating with health care providers within the hospital as well as in the community.
Practices professional nursing and promotes patient advocacy in accordance with The Nurse Practice Act and the ANA Code of Ethics for Nurses. Maintains a current Ohio Nursing License.

Responsibilities- Actively collaborates with the attending practitioner, the family and other members of the multidisciplinary team to coordinate an individualized plan of care across the continuum, communicates the plan to key stakeholders and facilitates a safe and timely discharge to the next level of care. - Utilizes clinical expertise and best practices to ensure high quality care is provided efficiently and cost-effectively. (Frequently)
- Maintains knowledge of regulatory requirements and ensures regulatory compliance for assigned area. Participates in emergency response preparedness as needed. (Occasionally)
- Participates in the training and development of new and existing staff members, including participation in peer evaluation. (Occasionally)
- Engages in "workplace community" by adopting changes to workflow in accordance with new processes or standards, participating in data collection or auditing, understanding and participating in department Quality Improvement initiatives, and representing the department at relevant meetings and relaying important information back to staff. (Frequently)
- Provides after-hour services as assigned through on-call availability. (Occasionally)
KNOWLEDGE , SKILLS AND ABILITIES REQUIREDMINIMUM QUALIFICATIONS:
1. Current licensure as a registered nurse in the State of Ohio required.
2. 3-5 years of nursing experience, preferably (but not required) in assigned clinical area
3. BSN Required
REQUIRED SKILLS:
1. Possesses excellent written and spoken communication skills.
2. Has the ability to communicate with health care professionals. Demonstrates diplomacy, tact and a professional demeanor
3. Working knowledge of CMS and other review agency standards.
4. Song organizational and interpersonal skills.
5. Demonstrates ability to effectively collaborate and delegate tasks.
6. Possesses ability to relate to diverse age and demographic backgrounds. .
7. Computer literate and working knowledge of software applications (word, excel).
8. Professionally appropriate analytical and organizational skills required.
MINIMUM PHYSICAL REQUIREMENTS:
- Bend/twist; squat/kneel; climb stairs/ladder; reaching above shoulder; lifting/carrying 0-20lbs; pushing/pulling 0-25lbs; work environment may include contact with communicable diseases and/or pathogens/patient equipment/machinery/electricity: OCCASIONALLY - 1-33% of the time.
- Standing/walking; driving; vision: depth perception/color vision/peripheral vision - FREQUENTLY - 34-66% of the time.
- Sitting; flexing/extending of neck; repetitive hand/arm use; computer skills; audible speech/hearing acuity; seeing - far/near; problem solving/decision making/interpreting data - CONTINUOUSLY - 67-100% of the time.
