Job Description
Complete a comprehensive assessment when this responsibility is delegated by the program director;
Participate in the development of the individual treatment, rehabilitation, and recovery plan and the updates;
Provide individual, group and family psychotherapy according to the client's individual treatment, rehabilitation, and recovery plan;
Communicate with the Program Director and psychiatrist regarding the clinical needs of the client as necessary;
Monitor, supervise, and oversee the program's daily treatment and activities in the absence of the Program Director as assigned by the Program Director;
Assist with aggressive discharge planning; and
Maintain a maximum staffing ratio of 1 to 8 clients.
Ensures all residential clients have active Medicaid and AABD.
Help consumers with SSA income.
Complete DLA-20 data for each consumer on caseload.
Complete re-authorization for assigned clients.
Will attend program and agency meetings.
Will be responsible for all MHB updates for assigned clients.
Qualifications and Education Requirements
Master’s degree in Education, Social Work, Public Administration, or related field preferred. Bachelor’s degree required.
Experience with State of Nebraska Medicaid Standards to include fraud, waste, and abuse standards.
Experience and confidence identifying and implementing evidence-based practices.
Ability to work flexible hours.
Demonstrate strong written and oral communication skills.
Illustrate proficiency in computer skills, especially using Gmail, Word, Excel and customized databases.
Be able to work in and adapt to the culture of a multi-cultural, community-based organization.
Commit to maintaining confidentiality.
Understand the Agency’s mission and culture and the ability to maintain appropriate boundaries with staff and clients in all circumstances.
Employment is contingent upon successful completion background checks.
Demonstrate inter-personal skills, ability to relate well with persons from a variety socioeconomic and cultural group.
Ability to lift supplies up to 25 lbs. to waist height.
Reliable transportation, valid Nebraska Driver’s License with a clear driving record and proof of insurance.

Will run 3-4 clinical groups per week.
Will see 5-6 clients per week.
Will attend meetings as assigned.
Will complete comprehensive assessment within 14 days of admission.
Will complete an initial treatment plan within 24 hours of admission and subsequently a comprehensive treatment plan needs to be completed within 30 days of admission.
Will review and update treatment plan every 30 days with the Program Director.
Will review treatment plan and discharge plan with each assigned client weekly.
Will create a transition plan within 30 days of admission.
Will create a relapse prevention plan within 24 hours of admission.
Will complete log notes within 48 hours of session/group.
Will complete discharge summary with client 7 days prior to discharge date.
Will assist clients in contacting SSA and DHHS of Nebraska to ensure benefits are active.
Re-authorizations will be completed in a timely manner when requested.
Submit MHB updates as required by designated MHBs (7 days, 14 days, 30 days, annually).
