The Lung Nodule Navigator and clinical coordinator manages the daily operations and workflows of the Lung Nodule program in conjunction with physician(s) and administrative leadership. The focus of this role will be to manage and follow up on lung nodules found through various avenues including but not limited to high-risk lung screenings, incidental findings in the Emergency Room and inpatient admissions. The Lung Nodule Coordinator will coordinate the next level of care for patients with lung nodules and assist patients, clinicians and program stakeholders in the collection, reporting and presentation of program outcomes. This role as Clinical Coordinator encompasses patients within the Charleston Market which will include TMC, SMC, CMC and three free standing ED’s. Also serves as resource for startup of programs within South Atlantic Division as requested.

Job Summary and Qualifications

Streamline care coordination and paperwork by assisting patient(s) with scheduling appointment(s) and exam preparation.

Support providers and assist patients in understanding their treatment options and resources available (e.g. Smoking Cessation and Pulmonary Rehabilitation).

Empower patients to remove barriers with issues of insurance, transportation, child care, financial resources in conjunction with internal and external resources (ex. Social workers, Financial Counselors).

Conduct follow-up conversations as needed with all patients and communicate concerns, changes, or social needs in patient health to appropriate MD or other appropriate care providers.

Conduct outreach to referrers, providers and other medical professionals.

Track and document interventions and outcomes.

Attend patient care planning conference and other meetings as necessary.

What qualifications you will need:

RN License or Registry in radiology required.

Minimum (2) or more years of clinical, healthcare related experience required

Preferred candidates will come from backgrounds in:

· program management or program development,

· patient/provider relations,

    · clinical roles in Radiology, Pulmonology, or Oncology clinical education
