The Medication Room LPN administers medications and treatments, draws blood, provides participant and family teaching, and supports those activities necessary to the preservation of organization in the home environment. Participates in the communication of the participants condition to the IDT and the medical providers in the PACE Clinic.
- Provides medication reconciliation and communicates with facilities and the pharmacy
- Implements orders written by medical providers
- Communicates effectively to the IDT and to family and caregivers, including providing instructions regarding medications and treatment
- Assists medical providers with medication reduction
- Performs daily medication count and inventory tracking of clinic stock medications and narcotics
- Responsible for monthly inventory and record keeping of Emergency Cart
- Responsible for daily tracking of medication refrigerator and laboratory freezer temperature logs
- Tracks, monitors and provides reports to medical providers of participants receiving antibiotic therapies
- Performs weekly medication reconciliation rounds at facilities
- Performs blood draws and other laboratory testing, medical management, IV therapies, wound care, etc. as directed by medical providers
- Provides general nursing care to participants and documents test results and examinations in the EMR
- Observes, records, and reports participants condition and reaction to drugs and treatments to physicians
- Maintains the confidentiality of all company procedures, results, and information about participants, clients or families
- Handles potentially infectious specimens with appropriate biohazard precautions, and practices universal precautions
- Other duties as requested/assigned
- Graduate of an accredited Licensed Practical Nurse Program and licensed as a professional in the State of Colorado is required
- One year of health care experience in hospital, long-term or home health care setting with 1 year of experience with a frail or elderly population is highly desired
- Must be IV certified in the state of Colorado or willing to obtain IV certification within or immediately upon completing 640 hours (4 months) of employment. Certification will be required to remain eligible for employment. Failure to obtain certification within the specified timeframe will result in termination of employment
- Must be proficient with computer applications such as email, navigating the internet, using electronic timekeeping, and Electronic Health Records
- Knowledge of medical equipment and instruments
- Skill in applying and modifying the principles, methods and techniques of practical nursing to provide on-going care for participants
- Ability to perform phlebotomy functions
- Skilled in developing and maintaining department quality assurance
