Licensed Practical Nurse - Providence Home Health in Woodburn, OR and surrounding areas.

$3,000 Hiring Bonus for eligible external candidates who meet all conditions for payment - this is in addition to the fantastic benefits and compensation package offered by Providence that begin on your first day of employment.

Schedule:

Monday-Friday, 8am-5pm with occasional rotating weekends

Job Description:

The LPN provides quality care to patients in the nursing center and their homes under the direction of a Nurse Case Manager or Clinical Supervisor following the established nursing process and procedures, and in accordance with the Nurse Practice Act. Performs all duties in compliance with all applicable State and Federal regulations and in compliance with all regulatory bodies. LPN's working in Home Health require 25% to 35% of time on daily travel.

Required qualifications:

  • Education to meet certification, license or registration requirement
  • 2 years Patient care experience in a skilled nursing facility or medical clinic.
  • Oregon Registered Nurse License upon hire
  • National Provider BLS - American Heart Association upon hire
  • Driving may be necessary as part of this role. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request. See policy for additional information

Preferred qualification:

  • Experience in Acute Care, Home Health or Public Health nursing.

About the Team

Providence Home and Community Care (HCC) is a service line within Providence with over 8,000 caregivers and 2,000 volunteers serving nearly 200,000 people a year, in their homes, in clinics and in a variety of congregate living ministries across our seven-state footprint.

HCC provides a full range of services and support for people of all ages, especially those facing chronic or life-limiting illnesses. Our core competencies include restorative care, longitudinal care and care through the end of life.
