Work Schedule: M-F, hours vary based on clinic needs

Position Summary
To provide clinical and administrative assistance to the members of the clinical staff at primary health care clinic locations as assigned. The Licensed Practice Nurse (LPN) is a member of a health care team, which includes, a Provider, a Registered Nurse (RN), a Patient Care Coordinator, Licensed Clinical Social Worker (LCSW), and a Medical Assistant (MA) that has been assigned a panel of patients. Participation goal in this care team model is to provide patients with an ongoing relationship with the team and continuity of care through all stages of care.

Essential Functions Statement(s)
  • Ensures provider staff boxes in EHR are followed up and completed within a timely manner including hospital follow ups.
  • Addresses back office complaint issues from patients, vendors, clinicians, other staff, etc.
  • Recruits and reviews medical assistant (MA) job applications, sets up group job interviews, completes all new hire paperwork appropriately, and oversees placement of student Medical Assistants for externships.
  • Maintains ultimate responsibility for VFC monthly accountability and administrative oversight of the Patient Logs, Temperature Logs, and Vaccines for Children (VFC) program for primary health care locations. Maintains stock supply list count for private stock vaccines/injections.
  • Maintains Sample counts for clinics by checking expiration dates and following Drug Disposal Policy for disposing of expired medications with 2 witnesses. Ensures documentation is completed when samples are signed out by staff
  • Leads Emergency Medical Drills and maintains Crash Kit Logs at Primary Care clinics. Routinely checks for expiration dates/ and re-orders Crash Kit supplies as needed.
  • Completes pre-certification of medications and patient testing as necessary.
  • Maintains monthly logs for PAPs, liquid nitrogen, and autoclave (autoclave is completed daily, weekly, and monthly).
  • Completes PAR level supply list count monthly, discarding expired supplies appropriately, and orders adequate supplies monthly as needed.
  • Coordinate monthly 1:1 meetings with direct reports and co-facilitate monthly Medical Assistant Meetings to include continuing education.
  • Assists MAs at each primary care office with administrative oversight of the OSHA program. Leads OSHA walk through during site visits.
  • Participates in QA data collection and Performance Improvement initiatives including weekly alert closures and Pre-Visit Planning Sheets in order to close care gaps within Primary Care.
  • Assists in community events as requested.
  • Performs miscellaneous patient care related and administrative activities as delegated.

Work Experience
Required Experience: 1-2 years, or willing to train new graduate nurse if a good fit for Primary Care office
Preferred Experience: 2-3 years
Computer Skills: Must possess a general knowledge of, and ability to operate, general office equipment.
Certifications & Licenses: LPN; Current Basic Life Support (BLS); Annually Required Respiratory Protection Fit Test
Other Requirements: Leadership and clinical skills necessary to effectively meet the needs of the staff, and to meet the healthcare needs of the patients in a safe, professional and efficient manner.

Degree Level Required: Other (Vocational)
Practical Nursing Certificate Field of Study: Nursing- LPN level.
