Overview
The Licensed Practical Nurse is a licensed professional who provides direct and indirect patient care services to patients in their home environment or alternative setting. The LPN collaborates with the Registered Nurse to provide nursing treatments to patients according to the plan of care established. The LPN provides technical nursing interventions including infusion, wound care, phlebotomy, ostomy, catheter care, and other skilled interventions as ordered by the physician and within their Scope of Practice. The LPN instructs, motivates, and assists patient and/or caregivers in learning and improving ability to live independently at home.

Understands and adheres to established policies and procedures
Understands and assists in resolving concerns from patients in a positive and professional manner Relays concerns to the supervisor or designee Observes confidentiality and safeguards all patient related information
Notifies supervisor of problems in a timely manner and works to help provide solutions
Completes ongoing education requirements and demonstrates ongoing competency in required areas
Assesses the patient's environment with particular attention to safety to ensure the environment is suitable for the delivery of safe and effective care Assesses the patient support system and utilization of available community resources to ensure optimum level of care
Provides patient and caregiver education relating to diagnosis, medications, infection control, and safety measures in accordance with the plan of care and under the direction of the Registered nurse
Conducts medication reviews every visit and ensure patients and their caregiver, if any, receive information regarding the patient's medication regimen, including but not limited to frequency, duration, use, side effects, indications, and contraindications
Provides education to the patient and their family regarding medication administration, actions, side effects, etc. (as above). Provides written materials for patients and their families to supplement in home teaching when needed during the visit
Ensures compliance with state/local/payer requirements and that documentation reflects the care provided and meets company, regulatory and payer standards
May be asked to cover after hours, weekends, and holiday on call to meet the needs of patients
Requisition and management of equipment and supplies required to increase the client's function and independence as appropriate to care Demonstrate teaching skills and continued competence in performing therapeutic interventions for all levels of complexity in patient care
Instruct patient/caregivers of the safety measures appropriate to care
Maintain universal precautions and infection control standards to prevent the transmission of infections and communicable diseases
Appropriately disinfect/sanitize/transport equipment and supplies used for patient care; and use proper medical waste disposal techniques Recognition of changes in patient status and reporting appropriately
Provide clear, concise, and complete reporting of patient status in compliance with agency and regulatory standards
Attends staff meetings, patient care conferences, and in-services as scheduled
Reports all incidents per agency and regulatory standards
Follows policies and procedures according to agency, state, and regulatory standards
Use of safe body mechanics to prevent personal injury to self or patient
Functions within the Nursing Scope of Practice and Nurse Practice Act
Maintain confidentiality of patient related information and honor patient rights
Participate in the agency's QAPI program by communicating problem areas identified and recommendations to address problem areas; data collection; attendance at periodic QAPI meetings; and participation in performance improvement projects as requested
Other duties as assigned within scope of practice
Qualifications
Graduate of a state approved school of practical/vocational nursing
Current license in the state in which practicing
Current CPR Certification
Home health and/or hospice experience preferred
Ability to prioritize tasks and multiple projects
Proficient in the use of electronic health records
