Overview

At Marin Community Clinics, we are passionate about all things integrated behavioral health (IBH). IBH “blends care in one setting for medical conditions and related behavioral health factors that affect health and well-being. Integrated behavioral health care, a part of “whole-person care,” is a rapidly emerging shift in the practice of high-quality health care. It is a core function of the “advanced patient-centered medical home.” (AHRQ).

We know that 70% of primary care visits are rooted in psychosocial concerns, and that providing integrated behavioral care in a medical setting leads to better health outcomes. IBH is an evidence-based service model that integrates behavioral health services (psychotherapy, psychiatry, substance use disorder counseling, and case management) into primary care. As a “safety net” health center serving a largely Latinx and Spanish-speaking population.

Our goal is to provide mentorship and support bilingual (English-Spanish) MSW graduates who will complete the Fellowship to become highly skilled, culturally responsive, and trauma aware clinicians that better represent the communities we serve. We believe that a mentored Fellowship represents a critical component of growing the next generation of IBH clinicians and leaders.

The Fellowship is a two-year paid program designed to meet the requirements for licensure in the State of California. Fellows will be immersed in a medical clinic and obtain hands on training and experience working within an IBH Model. Fellows will become adept at assessment, individual and group treatment, case management, and consultation skills. There are two tracks available for graduates, one Child & Family-focused and the other as an “IBH generalist”. Upon completion of the Fellowship, Fellows will be competitive applicants for positions at Marin Community Clinics as well as other team-based clinical settings.

This position requires the provider to be on-site. This is a 2-year fellowship. The fellowship starts in August of 2023 and ends in July of 2025. Fellows will receive a review at the end of year 1, and Fellows in good standing will be invited to continue through year 2. Upon completion of the two-year fellowship, fellows may be eligible to apply for regular positions at MCC. The fellowship program does not alter the “at will” nature of employment with MCC.

The Associate Social Worker Fellow is part of a dynamic, passionate and dedicated health care team, collaboratively working with medical providers, nurses, medical assistants, receptionists and other members of the organization. The Fellow, under the supervision of a licensed clinical supervisor, provides assessment and treatment services to Marin Community Clinic (MCC) patients across the lifespan and with a wide variety of clinical presentations. Fellows will provide assessment, brief interventions, resourcing, and follow up as needed. Fellows will have the opportunity to co-lead a behavioral health group and will manage a panel of short-term, individual therapy patients. Fellows will also rotate with Complex Care and provide resourcing and case management services to patients within the complex care caseload.

Responsibilities

  • Provide culturally sensitive and trauma responsive evidence-based treatment to MCC patients (adults and children) with “mild to moderate” behavioral health needs, including: assessment, diagnosis, therapeutic interventions, short term therapy, crisis intervention and safety planning, coaching, referrals and case management through both in-person and telehealth modalities.
  • Provide assessment and triage of patients referred by PCP for Warm Hand Offs, who answer positively on screeners (PHQ-9, PEARLS/ACEs, IPV, etc.), or through quality improvement outreach.
  • Perform all duties as a Healthy Steps provider and actively maintain a caseload of high-risk children and families **Child and Family Track Only.
  • Attend all training requirements including clinical supervision, shadowing providers, training seminars and department meetings.

Qualifications

Education and Experience:

  • Recent Masters of Social Work (MSW) graduate.
  • Registered with the Board of Behavioral Sciences (must have ASW number before start date).
  • BLS Certification is required.
  • Previous clinical experience should include DSM V assessment and diagnosis and crisis intervention.
  • Experience with & commitment to motivational interviewing.
  • Experience using evidence-based intervention in brief treatment format.
  • Commitment to cultural humility required.
  • Experience in primary care environment preferred.
  • Knowledge of state regulations and standards related to client treatment, patient rights, and patients/patient confidentiality required.
  • Any prior experience utilizing Electronic Health Records (EPIC) a plus.

Required Skills and Abilities

  • Bilingual (Spanish/English)
  • Demonstrated ability to work effectively on a team, show initiative, flexibility and enhance a professional culture of trust and good will.
  • Must be computer literate and knowledgeable in Microsoft Office software programs, and how to conduct searches on the Internet.
  • Ability to see 4 patients per 4 hour shift.
  • Ability to work full-time, in person in Marin County, CA (Fellowship is not offered remotely).
