Description
Tri-Cities Chaplaincy guides our community members to live their best lives, even and especially through the end of life. We are the recognized source for expert guidance during a crisis, at and through life's end, and following a loss.
As a Hospice LPN, you will provide critical nursing care to terminally ill patients in the comfort of their homes. Your primary responsibilities will include assessing patient needs, administering medications, providing emotional support, and ensuring that patients receive high-quality, dignified end-of-life care. You will collaborate closely with other healthcare professionals and family members to create a supportive environment for patients during their final stages of life.

At Tri-Cities Chaplaincy, we carefully establish salary ranges for our positions by taking various factors into account, including education, certifications, knowledge, skills, experience, and market conditions. These ranges are tailored to match the diverse programs and services we offer. Our goal is to ensure fair compensation for candidates based on these factors while maintaining internal peer equity.
Pay Range: $28.79/hr - $44.62/hr
BENEFIT INFORMATION
Mileage Reimbursement: $0.67 per mile, for eligible positions.
Vacation: Accrual of hours each pay period based on hours worked.
Wellness Days: After the 90-day introductory period, eligible employees can convert up to two sick days into wellness days.
Annual "Yay Day": Full-time employees receive 1 paid day off every January to volunteer in the community.
Sick Time: Earned on an accrual basis, with a 90-day hold for new hires (usable after 90 days)
Paid Holidays: Ten recognized holidays with no waiting period for new hires.
Health Benefits: Opt-in for Low Deductible Health Benefits, Vision, and Dental starting the first month following your start date.
Health Benefit Options: Medical, Dental, Vision, Supplemental Life, Accidental Death and Dismemberment, GlobeLife Voluntary Benefits, and Life Flight.
Voluntary Supplemental Benefits: Pet Insurance, Globe Life, and Life Flight.
Agency-covered benefits: Basic Life Insurance and Accidental Death and Dismemberment at 1x Annual Salary, Long-Term Disability, and Short-Term Disability.
401k: You can start contributing right away and up to 5% employer matching after 1 year of employment for eligible employees.

Premium Pay: Bilingual in Spanish: Verified through ALTA staff testing and Approved Certifications: Premium pay upon review and approval.
Tuition Assistance: After 1-year of employment, eligible employees can receive up to $5,000 annually in higher education assistance.
Continuing Education Units: Tri-Cities chaplaincy utilizes Relias Learning and CAPC for employees to earn continuing education units for their credential requirements.
Certification and Training Opportunities: Employees can partner with their department director on professional goals that benefit the agency and themselves for certification and training opportunities.
Requirements
MINIMUM POSITION QUALIFICATIONS

  1. Current Washington State licensure as a Licensed Practical Nurse
  2. At least two years nursing experience

PREFERRED POSITION QUALIFICATIONS
  1. Previous experience in hospice or palliative care preferred.
  2. Bilingual in Spanish.

RESPONSIBILITIES AND ESSENTIAL FUNCTIONS
Patient Assessment:
  1. Conduct comprehensive assessments of patients' physical, emotional, and psychosocial needs.
  2. Regularly monitor and document changes in patients' conditions.
  3. Collaborate with the hospice interdisciplinary team to develop individualized care plans.

Medication Management:
  1. Administer medications and treatments as prescribed by the hospice physician.
  2. Monitor and manage pain and symptom control to ensure patients are comfortable.
  3. Educate patients and families on medication management and potential side effects.

Comfort and Support:
  1. Provide emotional support and counseling to patients and their families.
  2. Foster a compassionate and comforting environment to promote the best possible quality of life for patients.
  3. Assist with personal care tasks such as bathing, dressing, and hygiene.

Communication:
  1. Maintain open and effective communication with the hospice team, including physicians, nurses, social workers, and chaplains.
  2. Communicate changes in the patient's condition to the healthcare team promptly.

Documentation:
  1. Maintain accurate and thorough electronic medical records.
  2. Document all patient assessments, interventions, and conversations with patients and families.

Education and Training:
  1. Educate patients and families on end-of-life care, advance directives, and available resources.
  2. Provide guidance on managing symptoms and promoting comfort.

Safety and Infection Control:
  1. Follow all infection control protocols to ensure patient and caregiver safety.
  2. Maintain a clean and safe environment for patients.

Professional Development:
  1. Stay updated on hospice care best practices and regulations through ongoing education and training.
  2. Collaborate with other healthcare professionals to enhance your knowledge and skills.

Miscellaneous:
