Job Description
The Hospice Liaison’s (HL) primary responsibility is to advocate for the Medicare Hospice benefit and educate on services available from Bristol to members of the healthcare and patient community. The HL accomplishes this by developing community relationships and maintaining positive relationships with customers and referral sources within the Community. The successful HL will quickly respond to customer and community needs and describe the Medicare Hospice Conditions of Participation to increase awareness of available services and program requirements. This position develops, facilitates and coordinates the referral process to promote a seamless, timely and comfortable transition experience for referring providers, patients and their families. The Hospice Liaison frequently provides presentations of hospice philosophy and program requirements to an array of healthcare and community audiences.
REQUIRED EDUCATION & EXPERIENCE: • Must possess one (1) year of sales, marketing, professional relations and/or education training required (healthcare background preferred) • Bachelor’s degree in a related field preferred • Hospice experience a plus • Must have a high school diploma or equivalent • Must be at least 18 years of age
REQUIRED KNOWLEDGE, SKILLS & ABILITIES: • Must possess the ability to express with care and concern the hospice care philosophy and goals • Must possess the ability to communicate with the general public and represent Bristol in a positive manner • Must demonstrate an understanding of, and commitment to comply with, all applicable health care laws and regulations • Must possess the ability to articulate the Hospice Conditions of Participation and facilitate the transitions in care for hospice patients, family members, caregivers and other members of the healthcare continuum • Must possess the ability to distinguish between the HL role which does not involve clinical determinations and the role of the Bristol clinicians who have the sole authority to admit patients to hospice service • Must possess the ability to create relationships with referral sources, patients and families • Must possess the ability to manage time and provider contacts effectively to achieve desired outcomes • Must possess the ability to work unsupervised, exercising a high degree of integrity, discretion, independent judgment, self-motivation and resourcefulness
