Position Summary:
The Physical Therapist is responsible for directing, supervising, evaluating and providing physical therapy services to patients in the home as prescribed by the attending physician.

Position Duties:
- Conduct comprehensive physical therapy assessments to determine the patient’s level of function in areas such as gross mobility, physical status, ambulation, muscle strength and tone, splinting and joint range of motion
- Instruct patients and families on the use of prosthetic, orthotic, assistive devices and home exercise programs
- Supervise the physical therapy assistant and certified nursing assistant
- Participate with the staff, patient and physician in discharge planning activities; complete physical therapy discharge summary according to policy
Other Requirements:
- Advanced knowledge of composition, syntax, medical terminology
- Effective written and verbal communication skills
- Must be able to speak and write in English
- Unrestricted driver’s license
- Current automobile insurance according to Company policy
Licensing/Certification/Education/Experience Requirements:
- Graduate of an accredited Physical Therapy Program
- One (1) year related experience
- Home health experience preferred
- Current registered physical therapist license
- State driver’s license
- CPR certification
Physical Requirements:
- Ability to lift to 30 pounds
- Work requires walking, standing, bending, reaching, kneeling, pushing and pulling
