Essential Functions:
- Manages a caseload giving consideration to patient/family needs, travel efficiency and service priorities.
- Plans, implements and evaluates individual occupational therapy programs and diagnostic tests in conjunction with the prescription and/or the direction of a physician.
- Treatment planning in consultation with the physician and interdisciplinary team (IDT) to achieve goals for clients and their families.
Other Duties:
- Instructs patients and family members in supportive, preventive and therapeutic procedures.
- Provides consultation to other Agency staff concerning the application of professional principles and procedures to problems in patient care.
- Consultation with IDT members.
- Identify barriers to desired outcomes.
- Evaluate equipment needs to increase functional level.
- Accountable to the Director of Rehabilitation for attainment of standards of professional practice.
- Completes records and reports necessary for patient service and administrative requirements as determined by the Agency.
- Assumes responsibility for professional growth and maintains clinical competence.
- Establishes and maintains effective working relationships with patient, families, peers, supervisors and community agencies.
- Responsible for meeting performance standards including qualitative and quantitative standards, as stated by the Agency.
- Identifies the need for services of other health care disciplines or Agencies and initiates the appropriate referral.
- Communicates regularly with the Patient Care Manager or other health care personnel to develop coordinated plans for patient care.
- Supervises Home Health Aide in carrying out the established plan for care and evaluates the effectiveness of the service.
- Participates, as appropriate, in implementation of the Agency's Quality Improvement Program.
- Participates, as appropriate, in Agency educational activities, including new employee orientation and professional student clinical training and Agency staff education programs.
- Interprets Agency services and programs to individuals/families and personnel or other agencies.
- Participates as assigned in community programs.
- Carries out other activities relevant to therapy services.
- Complies with the Agency's Policies and Procedures. (Contract therapist also complies with contractual arrangement).
