Home Health Nurse

We are searching for a reliable and compassionate home health nurse to join our client in Kalamazoo, MI. The home care nurses responsibilities include performing patient visits and monitoring their conditions, conducting tests, and writing reports for the physician. You should be able to address all the caregivers concerns and make appropriate adjustments to the patients in-home management plan.

Home Care Nurse Responsibilities:

RN Case Managers will administer skilled nursing care to patients requiring professional nursing service per the physicians orders and under the supervision of the Supervisor of Clinical Services. Will also monitor patient recovery and compile reports for the physician.

The RN Case Manager will manage each patients care efficiently and effectively to achieve positive patient outcomes, and be responsible for the overall plan of care and the outcome of that care, including care delivered by other nursing staff (LPN, HHA) who will be directed / supervised by the RN.

Educating the family and other members of the nursing team and responding to any concerns and/or questions

Collaborating with doctors and other healthcare professionals to improve patient diets and healthcare

Traveling to patients homes and managing their care plans according to physicians

instructions.

Specific tasks may include administering medication and insulin, completing blood pressure, glucose, urine, and stool tests, inspecting wounds, changing dressings, and handling personal grooming and hygiene. Might also involve testing for muscle weakness, bedsores, and checking for any signs of infection.

Providing suggestions for improved healthcare to physicians and family members of the patient.

Maintaining an understanding of the developments in healthcare and attend workshops and lectures as required.

Home Care Nurse Requirements:

A Registered Professional Nurse with current License

2+ year acute care nursing experience and/or previous home care nursing experience is preferred

OASIS experience strongly preferred

Med-surge and wound care experience preferred

Associates Degree from an accredited college required; Bachelors degree preferred.

Additional courses in biology, anatomy, and science may be required.

Good observational and problem-solving skills

The ability to follow instructions but also to act independently if necessary.

Excellent communication, report writing, and active listening skills

Physical and mental strength, including empathy

A valid drivers license and reliable transport may be required.
