Form of workHealth Coach will have general responsibility for managing the Cabrini funded Healthy Living for Adolescent and Young Adult Program offered by SBH Health System at its Health and Wellness Center. The Health Coach will be responsible for outreach communications and activities for recruitment of program participants from SBH and other community organizations. The Health Coach will also oversee outreach communications provided by program peers and collaborate with the HealthPlex Fitness Instructors to develop promotional incentives and program orientation activities. The Health Coach will track attendance and other program metrics and submit monthly program reports to SrVP and Director of Grants Management as well as collaborate with them and other key stakeholders to compile the annual program evaluation report required by the Cabrini Foundation.
Requirements
Individual with a BA/BS and background in such disciplines as social work, health education, exercise science and nutrition. Demonstrated experience working with adolescents and young adults in such setting as camp, team sports, and after school programs.
