Job description

Queens Community House (QCH) is a multi-site, multi-service settlement house that serves the diverse population of Queens through a broad network of programs and services for Children and Youth, Adults and Families, and Older Adults. The Family Support Coordinator (FSC) is a key member of QCH's Family Support Initiative in East Elmhurst and Corona. The FSC responds to student self referrals and requests coming from families in the community and through referrals from other QCH programs and provides comprehensive case management support aimed to help the household progress toward greater stability. The FSC will assess incoming requests; work together with families on the development of an action plan; connect them to benefits screening, housing assistance, mental health services, financial counseling, and other valuable resources; help families advocate for their rights with the DOE and other public entities; and provide follow up support to ensure that the household's objectives are being met.

Responsibilities
  • Provide a comprehensive and integrated case management service delivery model that promotes individual, household, and community well-being and helps families who are facing a complex array of challenges to develop and implement a comprehensive family plan.
  • Provide short-term case management to individuals and households. If individuals and households need intensive support, provide care coordination over an extended period to support households' sustained engagement in external services and progression toward family and individual thriving.
  • Provide Earn Benefits Online (EBO) services. EBO is a comprehensive and customizable casework-facing screening tool which serves as a "one stop shop" for clients, providing a fast, accurate, and confidential screening for a range of critical public and private benefits and resources, such as health insurance, food assistance, tax credits and energy assistance.
  • Assist with community engagement, relationship building and advocacy by building and maintaining collaborative relationship within our community centers and programs at QCH's school-based sites.
  • Participate in and actively contribute to cross-department meeting to support agency wide assessment, planning, and interventions in support of our families and neighbors i.e. weekly Family Support meetings and monthly Counseling meetings.
  • Introduce households to the range of Queens Community House's existing programs/services, and offerings including: after schools, youth employment, teen centers, food pantries, college/career advisement, adult literacy, immigration services, Benefits enrollments, senior centers, early childcare centers, and more.
  • Document and track participant engagement through Salesforce database and provide punctual and comprehensive reporting when applicable.
  • Identify and build relationships with internal and external service providers
  • Participate in case conferencing calls with other QCH Family Support Coordinators and case managers in site-wide meetings involving assessment, planning, and communication.

Qualifications:
  • Experience working with families with young children and if familiar with current best practice.
  • Bachelors in Social Work, Psychology or a related field required MSW preferred;
  • Minimum of three years' experience providing case management services to individuals and/or families and is familiar with current best practice models of family engagement work.
  • MSW or related masters' degree preferred.
  • Committed to the multi-service, settlement house model.
  • Can work independently while maintaining appropriate communication with supervisor.
  • Works well in high-stress situations and is able to prioritize tasks.
  • Interpersonal and communication skills, strong customer-service orientation.
  • Approachable and able to earn trust and credibility.
  • Able to apply principles of logical thinking to a wide range of intellectual and practical problems.
  • Willing to travel within the network catchment area.
  • Strong knowledge of various computer applications (word processing, presentation software, Salesforce).

Status: Full-Time, Non-Exempt
Salary:$24.72 per hour
Schedule: Monday- Friday/Flexible Schedule and some nights/ weekends may be necessary
