JOB DESCRIPTION:
Working in the home/community environment you are responsible for the delivery of quality one on one services to consumers while implementing their goals as outlined in the Person-Centered Plan. Your care may also involve activities of daily living and communication with other members of the consumers’ multi-disciplinary team.
You are required to complete all documentation in accordance with state and program regulations, Aveanna Healthcare policies and procedures, and any applicable accreditation standards.
ESSENTIAL JOB FUNCTIONS:
- Performed under the supervision of a registered nurse, licensed practical nurse and/or appropriate supervisor, who will be responsible for the client care you
- Helps the client to maintain good personal hygiene including bathing, shaving, grooming, and dressing.
- Assists in maintaining a healthy, safe environment
- Assists the client with transfers and ambulation with or without an assistive device
- Provides medication reminders
- Encourages the consumer to become as independent as possible according to the nursing care plan
- Attempts to promote client’s mental alertness through involvement in activities of interest
- Gives simple emotional and psychological support to the client and other members of the household
REQUIREMENTS:
- High school graduate, GED or equivalent of education and training
- Clearances or Certification as required by state law and regulation
- Demonstrated proficiency in clinical assessments, documentation and compliance with aide care and policies and procedures
- Completion of all applicable health screening(s)
- A minimum of two satisfactory references and background screenings
- Valid Driver’s License and Acceptable MVR
PHYSICAL REQUIREMENTS:
- Must be able to speak, write, read and understand English
- Must have reliable transportation
- Occasional lifting, carrying, pushing, or pulling, up to 25 pounds
- Must be able to sit and climb stairs
- Must be able to sufficiently reposition patients
- Must be able to appropriately respond physically and mentally to emergency situations in the onsite or during transport, such as perform one-man CPR for a prolonged period.
