- Summary
The Transition Specialist delivers vital community outreach, education, and information services targeting individuals with intellectual and Developmental Disabilities. The role involves close collaboration with schools, families, and advocates to enhance the community’s understanding and utilization of Developmental Disabilities services. The objective is to prepare these youths for inclusive and supported employment upon graduation from transition programs.
Work is done under the supervision of assigned manager and is guided by procedures established by Grays Harbor County Public Health Department. The position offers a flexible work schedule, accommodating occasional evening meetings, events, and other responsibilities outside of standard business hours.
Candidates must pass a criminal background check. Strict adherence to confidentiality rules is required.
- Essential Duties
- Assist in the development of program goals and objectives; establish means of accomplishing objectives.
- Meets state reporting requirements for Developmental Disabilities services.
- Function as a content expert specific to the needs of the Developmental Disabilities community including relevant resources, data, pertinent RCWs, WACs, and DDA Best Practices.
- Collects and analyzes information related to community services in terms of available resources, service usage, and identify gaps in service delivery.
Contract Management- Provides and procures training opportunities for community groups and providers to meet the needs of consumers and families.
- Maintain frequent contact with program participants, program subcontractors, human service providers, schools, and government officials specific to ensuring quality delivery of services.
Community Outreach and Education- Clearly communicate the intent and purpose of complex guidelines, regulations, and program deliverables to contractors, other government entities, and those utilizing the services provided.
- Facilitate on-going discussions, meetings, and initiatives focused on educating others about the developmental disability community with a variety of audiences.
- Non-leadership participation in local and regional committees and initiatives for information sharing and gathering.
- Provide technical assistance and facilitate access to training on best practices for providers, partners, and stakeholders.
- Regular outreach to transition age youth/families.
- Solicits input from people with Developmental Disabilities, community members, contracted agencies, and government partners regarding needed services.
- Qualifications
Minimum Qualifications:
- Bachelor’s degree in public administration, business administration, social services, public health, or related field OR
A minimum of five (5) years of relevant training and experience in the administration and/or delivery of Developmental Disabilities services to a community or combination of education and experience which would provide the desired knowledge, skills, and training to meet the requirements for the position which includes knowledge of pertinent RCWs and WACs.
Licensure/Certification:
- Current Washington State Drivers’ License

Knowledge, Skill and Abilities:
- Legal mandates (RCWs and WACs) related to Developmental Disabilities Administration, HIPPAA policies and client-centered planning.
- Current evidence-based practices in the area of responsibility.
- The creation of excellent verbal and written communications.
- The use of Microsoft Outlook for electronic communications and scheduling.
- The use of Microsoft Word to create professional documents, including advanced functions.
- The use of Microsoft Excel to create spreadsheets and charts using formulas, references, linking, and other advanced functions understanding, interpreting, and applying relevant statutes, codes, policies, and regulations.
- Project and program development and implementation.
- Providing consultation and monitoring of a variety of people and agencies.
- Manage multiple priorities and prioritize tasks in order to provide timely responses to requests and deadlines.
- Apply federal and state laws and regulations within the scope of this work.
- Contribute to the preparation of grant proposals.
- Work with diverse community organizations.
- Maintain confidentiality when dealing with client, contractor, and personnel matters.
- Use computer applications relevant to the functions of the position proficiently.
- Receive, process, and provide complex visual, verbal, and written information.
- Communicate in a manner that demonstrates respect and professionalism.
- Maintain a professional demeanor under heavy workload and stressful situations.
Working Conditions:
Work is primarily performed in an office environment. Physical requirements include, sitting, standing, walking, reaching, pushing, pulling, a sense of touch, finger dexterity, ability to grip with hands and fingers, ability to speak and the ability to read. Occasionally, there is a need to lift up to 20 lbs.
