The Home Medical Equipment Liaison coordinates home medical equipment (HME) services for patients being discharged from a designated hospital. The Liaison works at an assigned hospital with hospital-based Care Managers, Patient Care Coordinators (PCC), discharge planners, and Medical Social Workers (MSW). The Liaison collects and prepares all necessary documentation required for a complete order; communicates with Access Services and other outside vendors to complete the coordination of the order. The position acts as a resource and educates hospital staff regarding the services offered by HME and contributes to the patient care plan as it relates to the discharge plan. The Liaison provides information to ensure a smooth transition for patients and their families following hospitalization.

The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.

Providence Home and Community Care (HCC) is a service line within Providence with over 8,000 caregivers and 2,000 volunteers serving nearly 200,000 people a year, in their homes, in clinics and in a variety of congregate living ministries across our seven-state footprint.
HCC provides a full range of services and support for people of all ages, especially those facing chronic or life-limiting illnesses. Our core competencies include restorative care, longitudinal care and care through the end of life. Our service lines include assisted living/skilled nursing/rehabilitation, home health, home infusion/pharmacy services, home medical equipment, hospice and palliative care, Program of All-Inclusive Care for the Elderly (PACE), personal home services/private duty care and supportive housing.

Required Qualifications:
- A criminal background check is required for this position, based on assigned duties.
- 2 years Experience in the home medical equipment field.
- 2 years Experience in HME customer service or HME insurance verification.
Preferred Qualifications:
- Bachelor's Degree. Or equivalent educ/experience.
- 1 year Experience with CMS regulations regarding DME.
