We're looking for a Complex Youth Shared Planning Meeting Facilitator (SHPC3) to join Region 3 to provide services for youth in night-to-night placements, and planning for adulthood. Together, we will work toward the common goal of eliminating child and youth outcome disparities based on race and ethnicity. The Opportunity:
The Complex Youth Facilitator will facilitate Shared Planning Meetings (SPM) that support and encourage active participation of families and serve as a subject matter expert on available services and placement options to youth with complex needs. This individual will elicit and value the child's preferences including where to live, go to school, services to receive, supports for safety, and who will be involved with their lives. Some of what you will get to do:

  • Elicit participants in SPMs to make critical team decisions regarding placement prevention, stabilization, removal, and reunification.
  • Ensure high-quality, family-focused meetings to create a safe placement decision that is the least intrusive for children.
  • Work as a multisystem expert working with DCYF offices to facilitate and/or be part of meetings to provide and discuss community resources that can support families to prevent out-of-home DCYF placement when not warranted.
  • Provide consultation and guidance on SPM and 17.5 meeting practice, principles, and document meeting outcomes to determine if program goals meet outcome expectations.
  • Administer and Monitor SPM and 17.5 meetings in cooperation with regional staff and other facilitators.
  • Develop processes and procedures for use of staff and the Family Team and implement safety plans.
  • Facilitate Shared Planning Meetings, 17.5 staffings, and may provide backup support for FTDM's and Permanency Planning Meetings.
  • Develop and provide training to staff and community partners related to the effective use of Shared Planning Meetings and 17.5 staffings.
  • Collect data related to outcomes from completed meetings to monitor program outcomes.
  • Obtain consent from youth and parents at the start of each meeting and document meeting decisions.

What we are looking for (Required Qualifications):

One year of experience as a Social and Health Program Consultant 2, Social Service Specialist 3 (formerly Social Worker 3), or Financial Services Specialist 4 or above.
OR
Two years of experience as a WorkFirst Program Specialist or equivalent work experience with automated eligibility systems.
OR
A Master's degree or higher in Social Work, a health or social science, public administration or a related field, and three years of professional experience in planning, administering, developing, or delivering social, financial, health, or chemical dependency treatment services programs.
OR
A Bachelor's degree in social work, health or social science, public administration, or a related field; and four years of professional experience in planning, administering, developing, or delivering social, financial, or health services programs. Additional qualifying experience will substitute, year for year, for the required education, except for the Social Work degree.

NOTE: Additional qualifying education will substitute, year for year, for the required experience, except for the Social Work degree. One year of the paid social service experience must include assessing risk and safety to children and providing direct family-centered practice services (strengthening and preserving family units). Permanency planning experience desired.

NOTE: A degree in Social Work must be from an educational program accredited by the Council on Social Work Education.

In addition to those required qualifications, our ideal applicant will also have some or all of the following:
  • Previous experience in working with youth with a complex trauma history, resulting in mental health and behavioral challenges.
  • Previously demonstrated effective communication and comprehension skills, both orally and written, with management, peers, clients, community partners, and the public.
  • Previously demonstrated ability to fully utilize the shared decision-making processes for making case plans.
  • Previously demonstrated ability to exercise independent judgement in devising own work methods.
