Description
AMHC is excited to announce our New School-Based Community Health Navigator position at the Jonesport School System.
The School-Based Community Health Navigator is a trained public health worker who acts as a bridge between providers and individuals to promote health, reduce disparities, improve service delivery; and advocate for individual and community needs.
The School-Based Community Health Navigator is responsible for promoting AMHC and other services by connecting clients and their families to tele-behavioral health and linking students and their families with other needed services. The School-Based Community Health Navigator must collaborate closely with care coordinators and clinical support staff to ensure that community members have access to services provided by the agency. The chosen candidate is expected to demonstrate the core values of AMHC by showing Integrity, Compassion, Excellence, Service, Collaboration, and Respect to co-workers, clients, and community members
Qualifications:
- A High-school diploma is require
- Prior experience in education or health care is preferred
Responsibilities & Expectations:
- Collaborate with students, patients, AMHC, and health services to establish Respectful relationships in order to promote Service focus care.
- Provide Compassionate, culturally appropriate health information to the community, staff, and partners.
- Role-model the Integrity of the AMHC team by demonstrating Excellence in record-keeping, communication, and all other aspects of the navigator's role.
Schedule:
- This is a 40-hour per week non-exempt position
