The Community Health Worker (CHW) provides support for the Hackensack University Medical Center (HackensackUMC), Center for Memory Loss and Brain Health, a program seeking to provide comprehensive, patient centered coordinated care for dementia and memory disorders in NJ. The Community Health Worker (CHW) will primarily work with the community , attend various events, conduct memory screens and help the community with services to meet health-related social needs. The Community Health Worker (CHW) is expected to work directly with providers and patients in the community as the need arises.

Responsibilities

A day in the life of a Community Health Worker at Hackensack Meridian Health includes:

  • Work with patients in the community, clinical setting and their homes to identify social determinants needs affecting their overall health and memory care needs.
  • Act as a liaison between Center for Memory Loss and Brain Health, primary care providers, and community resources on behalf of patients to ensure patient-centered coordination of care. Participate in community based organization team meetings and maintain relationships with the goal of encouraging and facilitating access to community resources.
  • Helping individuals, families, groups and communities gain access to resources, including health insurance, food, housing, quality care and health information
  • Helping people understand their health condition(s) and develop strategies to improve their health and well being. Delivering health information using culturally appropriate terms and concepts. Linking people to health care/social service resources and advocating for local health needs
  • Educate patients on the proper use of medical services and provide them information regarding effective medical management of dementia and self-care. Assists patients in understanding care plans and instructions. Motivates patients/clients to be active and engaged participants in their health and overall wellbeing.
  • Screens potential study participants and assists them in determining eligibility for recruitment into clinical studies or other forms of assistance.
  • Enrolls participants and measures program participant engagement and satisfaction. 
  • Assists clients in accessing health related services, including but not limited to: obtaining a medical home, providing instruction on appropriate use of the medical home, overcoming barriers to obtaining needed medical care and /or social services.
  • Administer patient surveys, data entry, tracking, scheduling, and monitoring of patient appointments.
  • Facilitates communication and coordinate services between providers and the clients/patients.
  • Travels to community locations, various agencies, client homes, and other outreach destinations.
  • Creates and nurtures partnerships with hospital staff, medical office staff and local service organizations to broaden support services for patients.
  • Attend relevant training as necessary to maintain professional knowledge.
  • Adheres to HUMCs guidelines and ensures the appropriate handling of patient sensitive information.
  • May work non-traditional hours based on operational needs and to meet the needs of the community.
  • Other duties and responsibilities as directed.
Qualifications

Education, Knowledge, Skills and Abilities Required:

  • High School Equivalent degree.
  • Minimum of 2 years of work experience in a related field or area.
  • Strong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse community.
  • Requires the ability to travel to multiple office locations; personal vehicle and automobile that is insured
  • Demonstrates ability to carefully set priorities, meet deadlines, and schedule time efficiently.
  • Ability to accurately type and effectively operate a computer and use a variety of common software programs including Microsoft Office.
  • Working knowledge of multi-system outreach programs related to health care delivery, clinical education, and health-related services.
  • Ability to communicate medical information to health care professionals and care coordinators over the telephone.
  • Ability to work well and maintain professionalism under occasional times of stress and pressure.
  • Demonstrates ability to follow oral and written instructions.
  • Demonstrates ability to work with other people through a cooperative effort.
  • Possesses strong interpersonal skills as demonstrated by compassionate, courteous, cordial, cooperative, and professional interaction with diverse groups of co-workers, external business partners, and the community.

Education, Knowledge, Skills and Abilities Preferred:

  • Bachelor's degree.
  • Minimum of 1 years of relevant work experience in the non-profit, government, human services, and health care field strongly preferred.
  • Spanish Fluency highly preferred

Licenses and Certifications Required:

  • Valid Driver's License from a USA state.
