The CNA- PRN-PT Shift is responsible for providing direct care to residents in a long-term care facility. The position requires the ability to work independently, as well as part of a team, and to communicate effectively with residents, families, and staff.
*Shift Differential is applied for all PRN Shifts.
Key Responsibilities:- Assist residents with activities of daily living, including bathing, dressing, grooming, and toileting
- Assist with feeding and hydration of residents
- Monitor and record vital signs and other health-related information
- Assist with mobility and transfers of residents
- Provide emotional support and companionship to residents
- Document care provided in accordance with facility policies and procedures
- Report changes in resident condition to nursing staff
- Participate in care planning and interdisciplinary team meetings
- Adhere to infection control and safety policies and procedures
- Perform other duties as assigned
- Current CNA certification in the state of Utah
- High school diploma or equivalent
- Ability to read, write, and communicate effectively in English
- Ability to work independently and as part of a team
- Ability to prioritize and manage multiple tasks
- Ability to maintain confidentiality of resident information
- Ability to lift and transfer residents, as needed
- Ability to work a flexible schedule, including weekends and holidays
