Clinical Team Lead, Housing Support Services (LPC, LCSW, LMFT)
Reports To: Director of DBHDD Programs, Southeast
Job Class: Exempt
Hours Per Week: 40


Summary:
As part of program leadership, the Clinical Team Lead, a Georgia licensed LCSW, LPC or LMFT will support the Program Manager in the daily operation and clinical supervision of Housing First (HF) teams providing field-based Medicaid billable Housing Supports, Case Management and clinical services in the Savannah area. The HF team’s goal is to move adults experiencing homelessness and struggling with psychiatric and co-occurring disabilities into permanent supported housing and towards independence, community integration and long-term housing retention. The Clinical Team Lead will oversee aspects of intake, assessment, treatment planning, service provision and clinical supervision that ensures stability and housing retention in scattered site housing in Chatham and surrounding counties. The HF teams consists of Housing Specialists, Case Managers, Peer Specialists and Clinicians utilizing the Housing First, Trauma Informed and Harm Reduction models of service delivery to clients with the Georgia Housing Voucher (GHV) in community settings.
Essential Duties:
Primary duties include, but are not limited to, the following:
  • Clinical Supervision – Collaborate with program leadership to provide supervisory support and clinical guidance of the team conducting field-based housing support services. Assist leadership in implementation of clinical program policies, procedures, interventions used to conduct needed Outreach, Engagement, Clinical and Housing Retention Supportive services provided to members in community settings. Will assist leadership to support and coach team in clinical interventions and crisis management skills during weekly team meetings and case conferences. Assist Program Management to ensure overall policies, procedures, and practices are in line with Step Up, Department of Behavioral Health and Developmental Disability (DBHDD) values and Medicaid standards.
  • Administration – the Clinical Team Lead will coordinate with leadership to organize intakes, assessments and treatment planning of members and assist the team in aspects of the Medicaid billing process. Will support the team to ensure referral census data and service provision logs are up-to-date for Medicaid billing and accurate reporting to senior staff and DBHDD funders. Will share responsibilities of 24-hour on-call coverage and supervision duties.
  • Compliance – The Clinical Team Lead will assist management team to ensure staff are completing all required documentation and assist in monitoring productivity in the Welligent Electronic Health Record and to perform periodic internal chart audits. Will collaborate with leadership and Step Up’s Compliance and QA Departments to ensure charting and documentation meet Medicaid billing standards.
· Human Resources –The Clinical Team Lead will provide supervisory support and provide feedback and evaluation on a regular basis in line with Step Up Human Resources policies. Identify staff development needs and activities. Work closely with the team to bring in training opportunities and identify areas of professional. The Clinical Team Lead will provide supervisory coverage when the Program Manager is unavailable.
  • Tenant Services – Ensure field-based staff are working with members utilizing best practices and in clinically appropriate ways providing Housing Supports that develop “good neighbor” habits and learn skills that lead to housing retention and community integration. Coordinate and participate in crisis management interventions with the Step Up team, program leadership and other community medical, mental health and substance use organizations. Communicate regularly with community providers.
  • Other Duties as assigned.
Skills/Abilities:
Demonstrated and thorough knowledge of Housing First, Trauma Informed, Harm Reduction and Client-centered philosophies of service delivery. Knowledge of homeless services, community resources, housing vouchers, continuum of care and housing authority processes in the Savannah area. Demonstrated leadership skills and ability to work effectively on an interdisciplinary team. Clinical experience, crisis intervention, understanding and commitment to Recovery Model philosophy. Strong computer skills and must demonstrate proactive communication skills and be a team player with a goal towards Step Up’s mission to end homelessness.
Qualifications:
· Licensed LMSW, LCSW, LPC or LMFT in good standing in Georgia with a minimum of 2 years-experience providing intensive mental health services to persons struggling with psychiatric disabilities, histories of substance use and trauma in community settings. Supervisory experience, knowledge of Medicaid Billing, the Housing First model and Housing Vouchers systems PREFERRED. Patience, creativity, flexibility, compassion, and sensitivity to persons with disabilities and other minority populations. Proficiency utilizing Microsoft Office, homeless services databases and Electronic Health Records is essential. Must have own transportation and current Georgia driver’s license and insurance with a good driving record.
SUPPLEMENTAL INFORMATION: Work with people experiencing homelessness whether on the streets, in shelters or other places of habitation or services, may present inherent challenges and difficulties such as: exposure to bed bugs or other infestations, unpleasant smells or odors, individuals who may have poor hygiene or unclean homes due to homelessness, mental health symptoms, or poverty. Additionally, in working with our members, employees may encounter instances of profanity, sexually explicit or derogatory language, or verbal or physical expressions of anger and trauma. Very rarely do these situations ever escalate to physical contact, and negative or derogatory communication patterns can often be negotiated successfully. All direct care staff are provided adequate training to develop skills to deescalate crisis situations that may arise and Step Up has established procedural safeguards for all employees to mitigate (but not eliminate) these inherent risks of employment.
PHYSICAL REQUIREMENTS: While performing the duties of this job, the employee is regularly required to sit; use hands and fingers; handle or feel; reach with hands and arms; talk; and hear. The employee is frequently required to walk, balance, stoop, kneel, and/or crouch. The employee must occasionally lift and/or move up to 15 pounds. Specific vision abilities required by this job include close vision, distance vision, peripheral vision, depth perception, and ability to adjust focus. Keyboard data entry required.
