Job description

California MENTOR, a member of the Sevita family, provides services for adults and children with intellectual and developmental disabilities and other complex challenges, including autism spectrum disorders.

Case Management - Parent Partner
Do you have experience working in the mental health field as well as personal experience as a parent or guardian of a child who's been involved in the care system? Bring your commitment and empathy to a team-based workplace that puts people first.

As a Parent Partner, you will help the company achieve its goal of providing quality service by building supportive relationships with parents and caregivers. Provide guidance to parents/caregivers who have children being served through the system, sharing and relating your own personal, prior experience of having a child served in the system to help foster a relationship of trust and support between the parents/caregivers and the treatment team.

Provide advocacy and liaison services between the clinician, treatment staff, community agencies, services, families, and children to ensure that families are properly supported through the process of having children involved with the system of care, probation, or mental health systems, and utilizing any of the company's services. Participate as a full and active member of the family team, assisting the family in bringing up and discussing difficult issues at team meetings. Facilitate linking families with appropriate community services; follow-up, consult, and collaborate with community agencies to ensure they receive appropriate services.

Lead or co-lead individual and parent counseling groups providing education, consultation, or other needed services for current and discharged families. Attend and participate in trainings, treatment planning meetings, court review hearings, team decision-making, IEP, and other meetings to provide support and information to the teams and families. Maintain contact with families to best meet their needs, including visiting families' homes and initiating contact as soon as an individual is placed; provide support and information to allay families' concerns while offering encouraging feedback to those reconnecting to their community. Maintain comprehensive documentation as it pertains to record-keeping and state-specific requirements.

Qualifications:

  • High school diploma or equivalent required.
  • Two years of experience in a mental health setting strongly preferred.
  • Prior personal experience as a parent or legal guardian of a child who has been involved with the system (DCFS, probation, Regional Center, Dept. of Mental Health or the like) required. Probation and/or DCFS case must be closed for a minimum of one year.
