We have an exciting opportunity for an experienced Registered Nurse (RN) Care Manager to join our growing Population Health team supporting our Biddeford and Sanford, Maine ambulatory practices. Once oriented, two days per week will be remote. The other 3 days will float between the two practices.
Knowledge of EPIC and/or other electronic medical record systems is a plus. As part of a multi-disciplinary care coordination team, the RN Care Manager will work with patients who have primarily medical needs. These include helping patients get access to important resources that improve their ability to engage in care and meet their quality of life goals.
The RN Care Manager will review the current status of the patient, provides education regarding condition self-management and assess Social Determinants of Health (SDOH) related barriers and make referrals to community resources as needed.
As part of a team, the RN Care Manager continuously consults on the patient care plan to ensure that the interventions appropriately match the patient’s level of activation and helps to remove barriers to successful progress toward achievement of goals.
This is a 40-hour position working five 8-hour shifts during the Monday-Friday work week. Typical hours will be 8am-4:30pm, but there is some flexibility.

- Position is responsible for providing interventions designed to ensure the coordination and continuity of care and services for at risk patients with complex chronic medical, social, +/ or behavioral or mental health illness over time.
- Required Minimum Knowledge, Skills, and Abilities (KSAs)
- Education: Strong background in ambulatory care prevention, health promotion, condition management, case management and discharge planning.
- License/Certifications: Current, unencumbered Maine RN license required. Current BLS certification.
- Experience: Minimum of three years of direct clinical care required. Knowledge of Microsoft Office (Excel, Power Point, and Microsoft Word), Outlook (email), and internet familiarity is required. Knowledge of EPIC and/or other electronic medical record systems preferred.
- Knowledge of nursing processes, health care systems, structure and functions and community resources. Knowledge of medical record documentation requirements and federal/state laws related to release of health care information.
- Excellent communication, interpersonal and customer service skills.
- Skill in accessing and evaluating the available resources to meet a patient’s needs.
- Proficient computer skills with knowledge of EPIC or other related EMR.
- Ability to communicate clearly and effectively in various manners with patients.
- Ability to read & interpret physician’s orders and notes.
- Ability to maintain a professional manner even under stressful situations.
- Ability to maintain patient confidentiality.
