The Care Coordinator is an experienced clinical professional who provides guidance and education to patients and families. The Care Coordinator is responsible for health management and coordination of care for a designated population of patients and exhibits a deep understanding of each patients medical needs, insurance coverage, health literacy, and other socioeconomic factors that may impact their opportunity for wellness. BSN OR BA/BS with MSN OR ADN with BSN obtained within specified time after hire and 2 years or more of experience in Nursing. Certification(s) required: Current American Heart Association or American Red Cross Basic Life Support (BLS). Master's Degree in Nursing. 1 year or more of experience in Case Management. National Certification in Case Management.
