Job description

The mission of the Colorado Coalition for the Homeless is to work collaboratively toward the prevention of homelessness and the creation of lasting solutions for homeless and at-risk families, children, and individuals throughout Colorado. The Coalition advocates for and provides a continuum of housing and a variety of services to improve the health, well-being, and stability of those it serves.

$5,000 sign-on bonus (prorated based on weekly schedule)

The Colorado Coalition for the Homeless seeks a Behavioral Health Clinician II to work on the Housing First Community Support Team, which is designed to provide effective supportive housing, outreach, case management, and co-occurring treatment in order to enhance the health, safety, and well-being of the people we serve. Team members engage clients using strength-based methodologies to establish an alliance to ensure that the consumer is respected and supported to reach their identified goals. This position is responsible for the delivery of clinical-based services to individuals who have experienced homelessness. Duties may include individual and group therapy, crisis response, case management, benefits acquisition, care coordination, and linkage to resources. These services will be provided primarily outside of a traditional office space and occur most often in the community. This position is located at 2111 Champa Street Denver 80205 and reports to the Program Manager.

LSW/LPCC - $28.85 - $33.17 | Hourly

LCSW/LPC - $68,000 - $78,200 | Salary - Fully Licensed candidates are eligible for a $5,000 sign-on bonus (prorated based on weekly schedule)

Qualified candidates are eligible for the National Health Service Corps (NHSC) Loan Repayment Program. Licensed health care providers may earn funds towards student loans in exchange for a two-year commitment at an NHSC-approved site through this program.

NOTE: In order to ensure that our consumers receive the best possible care the candidate chosen to fill this position will be required to complete our internal credentialing and privileging process prior to a start date. The credentialing and privileging timeline is dependent on the selected candidate's submission of documents necessary for clearance. Please note that a start date will be scheduled once credentialing and privileging is complete.

Essential Job Functions

  1. Conducts intake screening and psychosocial assessments, psychotherapy, and crisis intervention as deemed appropriate by the team. Communicates clinical assessments, interventions, and ongoing client contact through timely, thorough record-keeping, contact forms, and verbal communication.
  2. Maintains a caseload including some court-ordered clients, monitors progress, and may report client outcomes during weekly court sessions.
  3. Maintains an additional caseload of voluntary clients, monitors client progress by regular, ongoing contact with individuals receiving services, and contact with collaborative service providers both external and internal to CCH.
  4. Works collaboratively as part of a Community Support Team, completing functions necessary to this role.
  5. Advocates for the needs of individual clients as well as for systemic change, whenever needed for the overall benefit of homeless individuals.
  6. Attends and actively participates in daily HF meetings that are clinical and administrative in nature to coordinate a high level of client care with team members.
  7. Works collaboratively with staff of all agencies to provide high-quality service. Reports back to referral source in a timely fashion to inform them of client disposition and additional services needed.
  8. Accesses services and situations for both immediate and long-term needs of clients, including identification (ID) information, transitional and permanent housing, income maintenance, insurance benefits, as well as comprehensive health, mental health, and substance treatment services.
  9. Develops core competency in the electronic health record and uses as directed.
  10. Record all clinical services in a timely and complete fashion in the EHR
  11. Maintain up to date records in EHR
  12. Record telephone and other clinical communications as needed in the EHR
  13. Manage tasks, PAQ, and scanning as needed to have a complete and current chart
  14. Maintain accurate information regarding client's insurance status and complete CCARs/DACODs and other clinical authorization documentation as needed
  15. Utilize the EHR and any future upgrades as directed

  18. Develops an individualized treatment plan for each client that includes all activities and services that are essential to meet the specific needs of the client. Monitors, evaluates, and updates the services listed in the treatment plan for their effectiveness in a timely manner.
  19. Works with specific assigned clients related to service planning, documentation of progress, service tracking, and outcome evaluation.
  20. Serves as a liaison to the Housing First representative payee for clients receiving income maintenance benefits who need such assistance.
  21. Performs necessary steps to access appropriate housing for clients.
  22. Transports homeless individuals using a CCH vehicle and an approved personal vehicle.
  23. Participates in crisis intervention for program clients, including assessing for and assisting with placing mental health holds when clinically indicated.
  24. Collaborates and coordinates with internal and external programs to facilitate client referrals and to ensure continuity of care. Serves as a liaison with collaborating agencies and other teams within the organization, establishing and maintaining effective working relationships with organizations and groups providing services to our target population.
  25. Cooperates fully with research component of reporting requirements for external stakeholders.
  26. Performs crisis management activities to ameliorate crisis situations for the client. Assists others with work that needs to be done at moments of urgency or crisis.
  27. Must carry a cell phone and be accessible when in the field to teammates, HF program clients, and community partners.
  28. Navigates the Social Security, CICP, Medicaid, and Medicare systems on behalf of clients.
  29. Completes Medicaid authorizations, HMIS data entry, as well as any other reporting requirements necessary to obtain, and sustain funding and provide services for individuals served.
  30. Meets formally with the HF Program Manager for ongoing individual and group supervision.
  31. Maintains current and complete COVID-19 vaccination(s), as defined by the Centers for Disease Control and Prevention and local health authorities.
  32. Performs other duties as assigned.


Administrative Functions
  1. Understands and maintains client confidentiality and HIPAA privacy practices.
  2. Completes Medicaid authorizations, HMIS data entry, as well as any other reporting requirements necessary to obtain, and sustain funding and provide services for individuals served in a timely manner.
  3. Provides in-person walk-in coverage on assigned days to respond to clients who walk in or call needing urgent services, as well as other in-person or telephonic requests for information or services.
  4. Provides 24-hour on-call phone support for HF consumers on a rotational basis.
  5. Attends daily team meetings.
  6. Must carry a cell phone and promptly respond to urgent job-related needs when working in the community.
  7. Consults with team members regarding how best to provide effective and appropriate services to clients.
  8. Meets formally with the HF Program Manager for purposes of collaboration, clinical review, documentation review, and supervision as well as informally for ongoing consultation and information sharing.
  9. Expands and improves case management skills through formal trainings, reading, and on-the-job experience.
  10. Prioritizes multiple job responsibilities to produce a high volume of work in a short period of time.
  11. Performs other duties as assigned.

Qualifications Summary

  1. Master's degree in social work, counseling, or similar field. Licensed as a LCSW/LPC preferred. If not licensed, must be already registered with DORA as a candidate for licensure (i.e. LSW/LPCC).
  2. Experience working or volunteering with the homeless population preferred or experience in a related direct human service field working with underserved populations.
