SalaryAs an Assessor Reviewer, you will travel locally to conduct in-person assessment interviews. Formal documentation of assessment notes may take place from your home office.
Essential Functions
- Perform face-to-face and/or virtual assessments of persons with disabilities.
- Some contracts may require collaborating with healthcare providers and ancillary service providers to create or validate the appropriate treatment plan.
- Document assessment results for the individual member status or negotiate service plans as needed.
- Provide ongoing education.
- May communicate with all health care providers to ensure appropriateness of care.
Requirements
- Two or four-year degree in a health care or related field.
- A minimum of two years of experience in working with persons with disabilities.
- Case management experience preferred.
- Maryland LON contract states assessors need three years of experience within the last seven years performing LON assessments.
- Required to pass and maintain a SIS certification (Telligen will provide).
- Reliable transportation is required
- Proficiency in written and spoken English is essential.
- Experience in human services is strongly preferred.
- Experience with assessments is a plus but not required.
