Job Description
At RHG, APP’s provide general and preventative care, conduct check-ups, diagnose and treat illnesses, order lab tests, and prescribe certain medication for our patients. RHG APP’s are more than just providers, they are mentors and educators, ultimately serving as champions for our patients. In this role, APP’s perform the very important function of educating our patients about preventative care and prescribed treatments.
RHG utilizes an integrated care delivery team-based approach in our clinics. Our APP’s are part of a care team that employs the use of front office administrative staff and Medical Assistants to deliver high quality primary care.
Key Responsibilities:
• Offer support for underserved/rural communities as a trusted primary caregiver: perform routine and detailed examinations, diagnose conditions, develop care/treatment plans, and prescribe certain medications
• With the support of RHG, proactively build your own patient panel, and serve as a patient champion for our communities
• Practice a mindset of managing patients for the long-term as opposed to just treating patients; educate patients (and families) about preventative care, available community resources, medical conditions, and treatment plans
• Collaborate with Medical Assistant and office staff to facilitate intake workflow to obtain information from patients and, in some cases, from families
• Collaborate with Medical Assistant to record patient histories and ensure medical records reflect symptoms, treatments, and administration of prescribed and over-the-counter medications
• Maintain and review patient records, charts, and other pertinent information: post test and examination results and document in a timely fashion
• Order diagnostics tests and evaluate results to determine underlying physiological reasons for patient symptoms
• Diagnose medical conditions and devise treatment plans designed to stop the progress of diseases and conditions
• Collaborate with other staff in the clinic to ensure patients have high satisfaction and receive the best care possible
• Collaborate with integrated clinic care team to share knowledge, create treatment regimens, and diagnose patients
• Assist in onboarding clinical staff (other NPs or PAs) and assist in providing competency checks for potential clinical staff
• Assist in training and onboarding Medical Assistants to your practice patterns within the RHG collaborative care delivery model
Qualifications and Experience
Minimum Qualifications:
• Graduate of an accredited school of nursing
• Graduate of an accredited Nurse Practitioner or Physician Assistant program
• Current Nurse Practitioner or Physician Assistant certification
• Minimum of 1-2 years of experience in the Nurse Practitioner or Physician Assistant role
• Primary/family care specialization is preferred
• Prior experience practicing in a rural setting is an asset, but not a requirement
