CHSI is a private, non-profit organization established in 1973 to serve the health needs of migrant & seasonal farm workers and their family members. Today, CHSI has expanded its services and provides mental & physical healthcare to all individuals in our communities. Our services are provided on a more affordable sliding fee scale to those who are underinsured or uninsured. We also accept ND and MN Medicaid, Medicare, and most private insurance plans. We currently operate four nurse-managed clinics, two mobile medical units, and two victim advocacy program centers in Minnesota & North Dakota.

WHO ARE WE LOOKING FOR?

We are searching for a Part-time Administrative Nurse lead to join our Rochester, MN team. In an Administrative Nurse Leadrole at CHSI, you would oversee a team of nurses and serve as their lead. The administrative Nurse lead is responsible for directing, organizing, and supervising the work of their nursing staff, which typically includes registered nurses (RNs), licensed practical nurses (LPNs), and Medical Assistants (MAs

Required Qualifications: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Bachelor of Science in Nursing (BSN) is preferred, but not required. The requirements listed below are representative of the knowledge, skill, and /or ability required.

Clinical Duties

  • Oversee/manage clinical support staff.
  • High-level oversight of vaccine programs.
  • Stay up to date with CDC recommendations/changes.
  • Communicate changes in vaccine recommendations with clinical staff.
  • Work with the Medical Director to update standing orders.
  • Ensure EHR is updated with vaccine changes (dose, name, new vaccine...)
  • Provide health care advice through telephone calls or the Patient Portal.
  • Develop and provide individualized and group counseling, guidance, and health instruction with a focus on disease prevention and health maintenance.
  • Provide nursing care during nursing visits based on provider orders, to meet the needs (physical, mental, and emotional) of patients. Demonstrate, understand, and perform triage activities for all appointed and walk-in patients and medical phone inquiries; using reasonable and astute clinical judgments in order to expedite the appropriate care and treatment of patients.

Supervisory Duties


  • Organize, and direct all clinical workflows of the nursing services in accordance with federal, state, and local standards, guidelines, and regulations that govern CHSI, ensuring the highest degree of quality care is maintained at all times.
  • Supervise RNs and LPNs at all four CHSI sites, including but not limited to interviewing, hiring, training, coaching, and performance.
  • Administer medications, injections, and vaccinations per facility protocols.
  • Carry outstanding orders for medication refills, ordering tests, diagnostic imaging, and labs to support the providers and notify providers of positive results as per facilities policy, helping to formulate a care plan and communicate all of the above to patients as needed.
  • Conduct newborn weight visits and lactation education.
  • Collaborate in the coordination of chronic disease management, including providing follow-up visits for basic disease management, including but not limited to Diabetes, Hypertension, Hyperlipidemia, and Asthma/COPD.
  • Follow up after specialty visits to ensure patients are following their current care plan.
  • Follow-up ED visits for specific diagnoses.
  • Follow up hospital discharge visits with a focus on medication reconciliation and follow through and education on the care plan and upcoming specialty provider visits. i.e., effects, complications, and patient usage considerations.
  • Perform a range of nursing procedures, including dressing changes, suture removal, oxygen administration, and basic wound care.

PHYSICAL ABILITIES

  • Ability to bend and crouch occasionally, sit continuously, and stand or walk frequently.
  • Ability to lift up to 50 pounds occasionally.
  • Ability to use hands/arms, feet/legs for repetitive fine to large motor skill movements.

EDUCATION & SKILLS

  • RN (BSN ) and current CPR certification from the American Heart Association are required. Excellent leadership skills including the ability to motivate & supervise others; and the ability to act discreetly and with diplomacy in relating to others, both internal and external to the organization, in personnel management and patient care services, are also required.
  • Knowledge and experience with primary care setting, including program development and quality assurance methodology; familiarity with PDSA and Lean methodology is highly preferred. A minimum of 3 years of ambulatory care, preferably in an FQHC setting, and 3-5 years of supervisory experience is a must.
  • The ideal candidate will have demonstrated ability and excellent skills to independently carry out assignments and nursing tasks; proficiency with Microsoft Word, Microsoft Excel, patient registries, and web-based tools and the use of an EHR; strong knowledge of medical terminology and relevant procedures; excellent written, electronic and oral communication skills; and strong critical thinking and problem-solving skills.
